LEARNING CONTRACT

This contract is entered into by and between; ___DOLDOLEA ANGELO.____________________________ fficially

enrolled in the courses offered for ______BTLEd IA 2A______________ with contact number and email address,

_______________09483219946_____ / _____angeloderecho1014@gmail.com___________________ hereinafter referred to as

student, and the respective instructors of these courses in the College of Teacher Education of Agusan del Sur State College of

Agriculture and Technology with contact numbers and email address (attached) hereinafter referred to as instructors. Bt

We agreed to the following stipulations.

A. STUDENT RESPONSIBILITIES:
1. Negotiable Items
I have chosen to complete the following actions:
1.1. I will spend a minimum of 1 hour a day reviewing my notes and working on assignments
1.2. I will check-in on time for scheduled online classes.
1.3. I will comply the requirements for the self-directed distance learning.
1.4. I will attend to scheduled modified face to face activities such as meetings, consultations, orientations, and laboratory
activities.
2. Non-Negotiable
2.1. I will achieve at least 85% of the assigned tasks reflected in the course syllabi of the courses I have enrolled in.
2.2. I will submit outputs on or before the deadline.

B. FACULTY RESPONSIBILITIES:
1. I will comply with the deliverables required of the course.
2. I will assist the students in performing their tasks required for online, modified face to face, and distance learning.
3. I will monitor the learning progress of the students with all integrity, and professionalism.
4. I will provide instructional materials needed for the course.
5. I will assess the performance of the students accurately and submit their grades on time.

C. GRADING SYSTEM

We agreed to the grading system shall be based on the accuracy, quality and timeliness of the expected outputs, stipulated in
the FTLA guidelines as follows:

The final rating shall be based on 40% Midterm Grade and 60% Final Grade weight allocations.

In witness whereof, this contract shall take effect on September 2021, until the end of the 1st semester A.Y. 2021-2022.
We have reviewed and agreed to abide with all the policies and guidelines stipulated herein.
