 Major Outcome/Goal: Cassie will learn to sit up independently.
 Because: Cassie can participate with peers in age-appropriate play
activities.
 What is already happening: When placed in a sitting position, Cassie can sit
for 1 minute
 We know we are successful when: Cassie is able to place herself in a sitting
position and maintain it for 3 minutes.
 Evaluation/Present Level: Cassie math skills are below average for his grade
level and below his expected level of achievement. He struggles with
multiplication and division and often becomes anxious in math class.
 Educational Need: to improve Cassie’s math skills to grade level
expectations
 Goal: Cassie will increase his math skills in the areas of multiplication and
