RREMINDERS for Test Preparation (for uniformity)

1. The text box format above is applicable to IDENTIFICATION and TRUE or


FALSE test types.
2. All exams in different subjects will be 60 items only for the first quarter
3. Types of exams that will fit in the Google form:
 Identification – Sentences / statements are in declarative form
 True or False – NOT modified
 Multiple choice – Sentences can be in declarative /or interrogative
but NO blanks.
Alphabetize the 4 choices following the format above
 Essay / Situational analysis / Problem solving / Application type
Put lines for essay answers.
All subjects will include these 4 types of exam. You decide yourself the
order of test type and the number of items for every test type.
4. Follow the test paper heading and the bottom signatories
You may put the school logo if you wish at the upper left margin
5. Use font size 12 and font style – Calibri.
Observe 1 inch margin in all sides: 1.15 spacing within text and double
spacing between test types.
6. In place of the TOS discussed by Sir Rafael during our webinar, I requested
Sir Kilala to devise a simpler one for the SHS that you will all adapt. You will
notice in the form that there are only 8 weeks, I excluded the 9 th wk for the
exam. This will allow you to construct test items for every weekly lesson or
topic. In that case, every lesson is represented with a certain number of
test items that may fall in any of the 4 types of exam. The test item
distribution sheet will also be submitted together with the test
questionnaire.

7. Do not forget to remove the text box (NO BORDER) before printing your
test questionnaire.
