Dear Sir,

I completed all the welding works by 6AM in 12 th Sep, after shifting all the personel to rooms and started
to CP8 by 9AM with one NDT technician, one Fitter cum Grainder, before started I talked with Lakshmi
narayana regarding work status, as he said one joint was fitted and strted root welding, for second joint
fitup need to wait till 1st joint completion because the fitter present at CP8 is not worthy and literally he
don’t know how to fix the 2nd joint.

As I reached CP8 by 11am on 12th Sep, the root was completed and DP test was taken after that, In DP
test on 12o clock position they found 80mm crack. After grinding the cracked portion and welded again,
again at same portion of pipe crack was found in Root, then after we decided to do pre heat then to for
welding, after the pre heat Root DP test was passed and completed other layers of welding and went
fitup the 2nd joint, it taken almost 3hrs to fitup the joint because of fitter who don’t know how to fix that
position, in between these 3 to 4 hrs he ran away from work and said he needs rest. The complete fitup
was completed by 8 PM on 12th. And I just went back CP6 location by 8PM.

Sir, As you said the entire welding of 2 nd joint completed by 12:19AM on 12th night, the radiography is
taken till 3:30 AM then after they developed the films and reviewed with TPI (BV), he accepted 2 films
out of 5 films because other 3 films contains some minor repairs (penetration, LOF,SLAG etc,). The
repairs were caused duo to overlap at one position and fitup error, the after they confirmed to do the
repair work by 6 AM in Moring of 13 th Sep. till evening they repaired the 3 portions of the pipe in
presence of HPCL and NOCPL ( Venkatesh and Narayana).

Sir, on 12th morning all works completed at CP5 after 48 hours struggle without rest to our CP5 location
welding team. I sent all the workers to room by 6Am after completion of welding and started to move
CP8 with one fitter, because as the information received from CP8 the 1 st joint completed and 2nd to be
complete, we think that all the works are going good at CP8, in afternoon from GR Engineers Mr. Raj
Kumar Called regarding de-mob of CP5 location welding team. As the work in CP8 location going good I
said okay to Mr Raj Kumar to De-mob from CP5.
