Four easy steps to get your detailed reports in Quizalize

1 Print and distribute the question papers and bubble answer sheets to your students.
Give each student a unique Student ID (eg. 10011) to enter on the bubble answer sheet then
start the assignment. The Student ID is how they will be identi ed on Quizalize.com when you
sync the results with your account.

2 Download the Quizalize Scan app from quizalize.com/scan-app

After the students have completed the assignment, collect all the bubble answer sheets.

3 On the Quizalize Scan app, tap on “Set up a new activity” and press “Scan”.
Scan the QR code at the top left of this page, then scan all the completed bubble answer sheets.
Once you are done, tap on the tick on the top right corner of the app screen. Click on ‘Upload to
Zzish’, login with your Quizalize account or sign up to create one, and select your class to
instantly sync the quiz data.

4 Log into quizalize.com on your computer or tablet to view your class results along with the
detailed reports from your Activity List and Mastery Dashboard.
Your reports will initially only show the Student IDs; to add names to your reports click on your
username on the top right of Quizalize and select “Edit your classes”. Click on the cog icon next
to each student ID to add their names. Enjoy your detailed reports!

8.G.A.3 Dilations, translations, rotations, and re ections
4 questions

1. A parallelogram with vertices at (0, 3), (2, 0), (4, 2), and (2, 5) is re ected over the y-axis. Which
vertex of the parallelogram will have the same x-coordinate before and after the re ection?
A. (0, 3) B. (2, 5) C. (2, 0) D. (4, 2)

2. On a coordinate plane, vertex A for triangle ABC is located at (6, 4) . Triangle ABC is dilated by a scale
factor of 0.5 with the center of dilation at the origin. The resulting image is triangle A′ B ′ C ′ . What are the
coordinates of vertex A′ ?
A. (3, 2) B. (12,8) C. (6.5,4.5) D. (5.5, 3.5)

3. Triangle ABC is graphed on a coordinate plane, as shown below. Triangle ABC is dilated by a scale factor of
2 with a center of dilation at the origin to create △ A'B'C' . What are the coordinates of the vertices of △
A'B'C' ?

A. (4, 4), (8, 8), (−8, 4) B. (4, 2), (8, 6), (−8, 2) C. (4, 4), (6, 6), (−2, 4)
D. (1, 1), (2, 2), (−2, 1)
4. Mia enlarged a plan for an outdoor stage. The original plan is shown below. She dilated the outdoor stage
by a scale factor of four with the center of dilation at the origin. Which ordered pair will be the coordinates of
one of the new vertices?

A. (2, 1) B. (32, 16) C. (8, 16) D. (32, 4)
