May we furnish you a copy of the Audit Report submitted before the Board of Directors by the Audit
Committee that conducted an audit of SCHHAI accounts for the fiscal years 2019 and 2020, the period
covering your term of office.

You may opt to submit your response or explanation relative to the audit findings for consideration of
the Board of Directors on or before ________________.

Due to some noted significant findings that need immediate attention, we further invite you or your duly
authorized representative/s for a conference on ___________ at __________, to discuss these noted
observations and to air whatever your comments on the Audit Findings are.

Please note that failure to submit a written response or explanation to the audit findings within the
specified date and/or failure to appear on the scheduled conference shall be deemed a waiver of your
right to be heard and shall prompt the commencement of appropriate legal action if found warranted
under the circumstances.
