NURSING HISTORY

A. Chief Complaints

Loss of consciousness, pain upon urination with burning sensation


and epigastric pain.

B. History of Present Illness

One day prior to admission, patient experienced nape pain,


recurrent chest pain at the right side and non-radiating with a pain scale of
8/10, back pain at the scapular area bilateral, epigastric pain pricking and
non-radiating with a pain scale of 8/10 .After that, patient had a sudden
loss of consciousness .It started on Aug. 21, 2011 at 3 pm and having his
siesta in his room when he suddenly felt the nape, back and chest pain
together with the epigastric pain which lasted for an hour. Later on he
decided to call his daughter on the other room to ask for help when he
suddenly loss his consciousness before his daughter arrived. After 10
minutes, when he woke up and his consciousness returned, his daughter
was already doing massage on his head and relief was felt, his daughter
told him that he must go to the hospital to have a further check-up but he
refuses to follow her advise because he was afraid that he might have a
severe illness and he was thinking that he may not survived considering
his age and the condition of his body. Last August 22, 2011 in the
morning, they decided to admit him in Adventist Medical Center-Iligan
(formerly called Mindanao Sanitarium and Hospital) due to recurrence of
his symptoms such as pain upon urination, nape, and chest pain at the
right side non-radiating with a pain scale of 8/10, and back pain at
scapular area bilateral and epigastric pain pricking and non-radiating with
a pain scale of 8/10.

C. Past Health History

The patient had experienced measles, mumps and chicken pox


during his childhood years however he cannot remember the specific age,
and hospital upon diagnosis and the treatment that was administered. He
did not experience any accidents or injuries and any serious and chronic
illnesses. Ever since he never experience to be hospitalized or undergo an
operation. He had undergone immunizations such as OPV, Hepatitis B,
and DPT during his childhood years. He had undergone eye examination
last December 2007 when he was experiencing blurring of vision. He
doesn’t take any medications or maintenance of vitamins. He only had
land travels from Iligan City to Marawi City to have a consultation on their
family doctor for the recent update of his health status.
