We understand that Mitacs does not make any representations or warranties of any kind, expressed or
implied, with respect to the award or the information concerning the participants. We agree to collaborate
with the applicant at our own risk and without any liability or responsibility for Mitacs. We understand that
we alone are responsible for evaluating the accuracy, integrity, quality and completeness of the application
from the other participants.

We hereby unconditionally and irrevocably release and discharge Mitacs and its affiliates, employees,
directors, officers and other representatives from any and all claims, damages, costs, losses and expenses
whatsoever that the participants, their affiliates, employees, directors, officers and other representatives
may have arising in connection with the travel.

I confirm that I have the signing authority to offer these services in support of the company’s travel to our
Incubator.
