I. DETAILS OF THE ACTIVITY:

Title of activity: BAYANIHAN, BAKUNAHAN National COVID-19 Vaccination

(2nd Dose)
Date of conduct: December 29, 2021
Place/Venue: Barangay Kematu, T’boli South Cotabato

II. Objectives:

General:
To increase COVID-19 Immunization coverage to the province before end of December
2021 to reach the goal of 70 to 90 percent herd Immunity.

Specific:
To augment Manpower complement of the Municipality in the identified vaccination
site of Barangay Kematu, T’boli South Cotabato.

III. Target Barangay:

 Brgy. Kematu, T’boli south Cotabato

IV. Summary

The IPHO vaccination team in partnership with the T’Boli rural health unit work force
facilitated a 1-day Bayanihan Bakunahan COVID-19 Vaccination Drive (2 nd dose) on
December 29, 2021 at Brgy. Kematu, T’boli South Cotabato.

The team arrived at the Barangay Gymnasium at 8:45 AM. The Activity started a
courtesy call with their Brgy. Midwife. Prescreening process were conducted after,
followed by Vaccination. From 800 expected vaccinees, the team only vaccinated
722 individuals wherein; 228 vaccinees received the 2 nd dose of Pfizer COVID-19
Vaccine, 483 individuals received their 2 nd dose of Sinovac shot and 11 vaccinees
received their 2nd dose of Moderna shot. After Vaccination, vaccinees were checked
and assessed for possible adverse reaction. Relief goods provided by the DILG
South Cotabato were distributed to the BHW’s thereafter.
