Forty year old female presented to us with
history of high grade fever with chills and abdomen
pain for last four to five days for which she took
treatment from the local prac oner.
She did not respond to that treatment and
started developing altered sensorium with marked
breathlessness and difficulty in breathing. Because
of all these complica ons, she was referred to
Pushpanjali hospital for further management and
care.
In the emergency, she was evaluated and she
was found to have low blood pressure, low oxygen
satura on and severe breathlessness. In view of all
above findings pa ent was intubated and put on
mechanical ven latory support, her hemodynamic
parameters were stabilized with the help of IV fluids
and vasopressor infusion and she was shi ed to
Intensive Care Unit for further management and
evalua on.

In the ICU she was further evaluated and
stablized. she was found to have dengue NS-1
an gen posi ve with very low platelet counts. Her
liver func on test was markedly deranged with
severe transamini s and high INR and deranged
kidney func on test. Inview of all above findings she
was diagnosed to have dengue shock syndrome with
MOD. She required very high ven latory support and
high vasopressor support. She was treated with
suppor ve care, platelet transfusion, IV an bio cs
and guided fluid therapy along with other suppor ve
care. She gradually improved. A er Five days she was
off vasopressors and her ven latory requirements
decreased. She was weaned off from the ven lator
on seventh day of her admission. Her platelets start
showing improvement and she did not require
further platelets transfusions. She was shi ed to
g e n e ra l w a r d fo r f u r t h e r s u p p o r v e a n d
rehabilita ve care. She was mobilized with help of
physiotherapist and her nutri onal plan was made
for home. She was discharged on day 10th of her
hospital admission with the diagnosis of dengue
shock syndrome, severe ARDS and mul organ
dysfunc on syndrome.
The clinical manifesta ons of dengue range
from self-limited dengue fever (DF) to dengue
hemorrhagic fever (DHF) with shock syndrome,
which may carry a significant mortality rate if
improperly managed. The risk of severe disease is
much higher in secondary rather than primary
dengue infec on.DHF is the most serious
manifesta on of DENV infec on and can be
associated with shock. The four cardinal features of
DHF include increased vascular permeability, fever,
h e m o r r h a g i c m a n i fe sta o n s , a n d m a r ke d
thrombocytopenia (100,000 cells/mm3 or lower). As
DSS carries very high mortality risk, early recogni on
of progression to severe disease and pa ents at
increased risk for severe disease is essen al, with
prompt ini a on of more aggressive therapy when

fever. His outside dengue test was posi ve. He had

We did a Laparoscopic Nephrectomy of Mrs. RD, fi y
nine years female pa ent. She presented to us with
pain abdomen and was found to have a large le
upper ureteric stone on evalua on. She wanted
surgery for the stone but CT suggested a Non
Func onal le kidney with pyonephrosis.
We convinced her that removal of kidney was
the right treatment for her. We performed Le
Laparoscopic Nephrectomy with three ports and
defunct kidney was removed via a small 3 cm
incision. Pa ent did very well post op and was
discharged on third day.
On follow up, she was very happy. I would like

Mr. Charanjeet, Fi y Four years old male,
known case of Liver Cirrhosis under regular
treatment from ILBS, New Delhi. He was scheduled
for liver transplant in next 1-2 months. He went

our emergency in altered Sensorioum and Gasping
condi on.

On further inves ga ons he was diagnosed
on a case of chronic liver disease, portel
hypertension, hepa c encephalopathy-grade IV and
------ syndrome. He developed cardiac arrest in the

now and has been discharge. He is fit for his liver
transplanta on is that he can lead a normal life.
