This sample evaluation process is based on a Performance category of an employee.
1. Employees should fill out the Evaluation Form.
2. Manager/Supervisor will review if employee’s answers are accurate based on the employee’s performance year-to-date. 3. Manager/Supervisor to identify what are the performance or behavior that needs to sustain and needs an improvement. This is based on the evaluation form and manager’s personal observation with his/her subordinates. a. Manager/Supervisor will provide feedback, action plans, and/or share best practices for employee’s improvement. b. Manager/Supervisor should apply SMART goal in providing feedback/action plan. c. Manager/Supervisor must take note the discussion and have it signed by the employee. (We will provide a SMART goal Template) d. Manager/Supervisor need to make a follow-up weekly or monthly to make sure that the employee is doing the action item.
