LICENSED PROFESSIONAL TEACHER
 
Instructions: The questions below are mainly used as a guide, student researchers can
add follow-up questions to draw out more useful data for a more in-depth analysis.
The teacher-respondent is free to answer in either Filipino or English Languages, or
both. But the analysis prepared by the student-researcher should be in the English
Language.
