Activity 1: Taking the family you know well as a subject (other from
your own), answer the following learning activities below:

a. Map or profile the family and its members using a different


assessment tool.

* Family Structure:

The first generation paternal side of the family is


Matriarchal because the father of the family dies at the age
of 22, which leads the mother of the children to support the
whole family.
The second generation of the family is nuclear family but the
father of the family died at 56 years old last 2019 so the
mother has supported the family since then.
The oldest and the middle child of the third generation has
their own family and the youngest is the only child that the
mother is supporting.

* Characteristics

Based on the map above all the first generation and second
generation families are all married. While in the third
generation only the youngest son is not married.

* Dynamic
In the first generation paternal side it is composed of 4
children, only the 2 children are alive.
Meanwhile the maternal side is also composed of 4 children,
all of them are still alive. Parents on the same side are
dead.
On the third generation, the first child is dead due to
miscarriage. The three sons are alive.

Socio-economic and cultural characteristics


* Family income and expenses

- Based on the Mother in the second generation, she is working


in their Barangay Hall as an DSWD worker. Her three sons are
now working. Her work is enough to support the bills and her
one son.

* Educational Attainment

- The mother of the second generation is a High school graduate


and her two sons are all high school graduates and her
youngest son is a senior high school graduate and will study
this year.

* Ethnic background and religious affiliation

- The religion of the family since then is Roman Catholic.

Home and Environment


* Home

- The home of the second and third generation is their own, and
the house they’re living in is also the house of the paternal
family in the first generation.

* Kind of neighborhood

- The first generation maternal side of the family lives in the


province, which is in Samar, meanwhile the second generation
paternal side of the family lives in the city.

* Social and Health Facilities

- The second and third generation are near to the barangay


health center and facilities because they live in an urban
area.

* Communication and Transportation Facilities Available

- According to the Mother of the second generation, due to her


sons having their own family, they don’t communicate with
each other that much, only her youngest son is with her. The
transportation in their place is easy and near.

* Health Status of each family

- According to the statement of the Mother in the second


generation, her parents both have high blood pressure.
- On the paternal side, both grandmother and grandfather have
high blood pressure.
- The second generation on paternal side, the two deceased
children both have high blood pressure.
- The second generation on the maternal side, the two daughters
in middle both have high blood pressure.
- The mother in the second generation had miscarriages because
of the overdue baby inside her womb.

Values, Habits, Practices on Health Promotion,


Maintenance and Disease Prevention
* Immunization

- The whole family had completed their vaccination, they also


had COVID-19 vaccine this year.

* Healthy Lifestyle Practices

- The second generation mother has a normal weight and body


composition, also her son has a normal body weight to its
height.

* Adequacy of Sleep and Exercise

- The mother works at the Barangay Hall during night until


midnight. But she has enough sleep for 8-10 hours from
morning to afternoon.

* Use of Promotive- Preventive Health Services

- We, the Nursing students of Jose Rizal University, are


promoting the family to eat healthy foods to prevent them
from developing diseases like High blood pressure.
- The family must refrain from eating saturated fats because it
can be a cause of developing the said disease.
- Enough exercise everyday to maintain the physical fitness of
everyone in the family is a must thing to do because it can
give a lot of benefits to their health.

* Health Needs

● Having a healthy diet, eating healthy foods like


vegetables and fruits and avoiding fatty foods
especially saturated fats like red meat and butter can
help prevent the disease.
● Being physically active is also a big help. Exercise
regularly to lose stored fats and to make bones and
muscles become more engaged in moving.
● Getting enough sleep can help your heart and blood
vessels become more healthy.
● Limit the alcohol intake because drinking irresponsibly
can reduce the possibility of heart attack and stroke.
● Avoid smoking. Smoking can increase the risk of high
blood pressure and heart attack.
● Adequate knowledge about the disease to know preventive
measures.

Activity 3: After experiencing conducting a family assessment


yourself, discuss the importance of collecting accurate and factual
information in setting your goals to help the family improve their
health and well being.

Everyday there is a great change or emergence of new diseases


and changes in health care needs. The demand for high quality
nursing care increases with each passing day. The quality of
nursing care services increases patient satisfaction and the
quality of health in general. Therefore, nurses have to provide
effective care for their patients by using up-to-date knowledge,
accurate data and factual information. As nurses do their duty
each day, it is only proper to give them the best accurate
information a patient can give so they can plan out an effective
care plan to provide for their needs. With this in mind, an
effective nursing care plan can only be effective with the help of
the patient. If the patient is lying, giving false information and
inaccurate descriptions, it can be detrimental for the patient as
they can be prescribed with the wrong medicine and they can be
treated incorrectly which can result in further damage or
endangering the patient’s life.
