 Freddy involved and injured

 Cable lying on the floor
 Power cables and chords not sticked to walls
 Office equipment’s wire not properly managed.

 Workplace not maintained in a safe condition
 No risk assessment process undertaken

2. CONSEQUENCES OF THESE HAZARDS + RISKS

 Freddy – senior staff member, slipped on exposed power cords & computer cables on
the floor
 Cables + cords presence is as a result of new staff members + the installation of new
computers
 Fell forwards
 Broke his wrist as a result of the fall
 Will need to take time off work to recover
 Possible of legal action because of unsafe working conditions
 Disruption of day to day activities
 Have many hazards and high potential risk with it.
 High risk to injury to all workers if worked in the recent working conditions

3. WHO IS IMPACTED BY THESE HAZARDS/RISKS?

 Employee who are working there are impacted by these hazards.


 They are very prone to big injury
 High probability of property damage
 Disruption of work

4. ASSESSMENT OF THE LEVELS OF THE HAZARDS + RISKS


 Acceptable
 The risk is acceptable because it can be controlled.
 There should be made some changes around workplace which will eliminate the
hazards.
 The level of risk can be brought to negligible level after some changes made on
 Proceeding with approval of change to supervisors

5. CONTROL OF THE HAZARDS + RISKS

Immediate control
 Cable lying on the floor should be removed immediately or managed so
that it does not lay on the floor
 Network switches should be put in the corner where it will be safe for
everyone and the cables channeling should be sticked to walls
immediately.
 Office equipment’s wires should be managed and kept in a place where
employees don’t have a contact to it

Management should address the hazards of the current working environment at first and
accident in the working environment.
 Management should inform everyone and make them aware to be careful until the
problem is resolved.
 Immediate action should be taken to Freddy.
 Management should give a survey to employee so that they what changes should be
made to make a safe working place
 Management need to take detail steps to address the found hazards and start
eliminating those hazards.
 Should have a proper report of hazards and plans to eliminate the hazards
 Standardize the reporting process. Educate all employees on this. Use posters,
intranet, forums, “tool-box” meetings for to eliminate future injuries.,
