TRAVEL DOCUMENTATION
° A travel documentation is an identity document issued by a government or international treaty
organization to facilitate the movement of individuals or group of people across international
boundaries, following international agreements. Travel documents usually assure other
governments that the bearer may return to the issuing country, and are often issued on booklet
form to allow other government to plays visas as well as entry and exit stramps into them.

HISTORY OF PASSPORT

- Passport is an important part of the Chinese Bureaucracy as early as Western Han and in Qin
Dynasty

- They required such details as age, height, and bodily figures

- The passport is known as "Zhuan"

- In Medieval Caliphate, they used a passport known as " Bara'a" ( "zakah" for Muslims and "jizya"
for Dhimmis)

- King Henry V of England is credited with having invented what some consider the first passport in
the modern sense.

- Earliest record found in 1414 Act of Parliament

1540: Granting travel documents in England became a role of Privy Council of England.

WWI: Europeans introduced border passport requirements for security reasons

1914: The British Nationality and Status of Aliens Act was passed clearly defining notion of
citizenship and creating a booklet form of the passport

PASSPORT

- A formal document issued by an authorized official of a country to one of its citizens that is usually
necessary for exit from and re-entry to the country.

- A license issued by a country permitting a foreign citizen to pass or take goods through its territory.

- A document of identification required by law to be carried by persons residing or travelling withing


a country.

GENERAL REQUIREMENTS:

- Personal Appearance

- Confirmed Appointment

- For NCR-based applicants: printed application form downloaded from DFA Appointment System

- Birth Certificate in Security Paper (SECPA) or Certified True Copy of BC issued by Local Civil Registrar
(LCR)
- Valid picture ID's and supporting documents to prove identity.

PASSPORT - IN CASE OF NO BIRTH RECORD

• If born in or after 1950 ( January 1, 1950 or after)

- Apply for delayed registration of birth at the local civil registry office located at the place of birth of
applicant

- Submit authenticated Birth Certificate from PSA and supporting public documents with correct date
and place of birth.

• If born before 1950 and below ( December 31, 1949 or below)

- Certificate of Non- availability of Record from PSA

- Notarized Joint Birth Affidavit of Two Disinterested Persons

- Any public documents with correct full name, date and place of birth

PASSPORT- MARRIED WOMEN/CITIZENSHIP

• For Married Women

- Marriage Contract or Original and Photocopy of the Certified True Copy of MC

- Report Marriage duly authenticated by PSA if married abroad

- Original and Photocopy of Commission of Filipino Overseas, Guidance and Counseling Certificate of
Attendance

APPLICANTS WHO AVAILED OF DUAL CITIZENSHIP UNDER RA 9225

- Identification Certificate of Retention or Re-Acquisition

- Oath of Allegiance

- Order of Approval

APPLICANTS WHO ELECTED PHILIPPINE CITIZENSHIP


- Identification Certificate of Election

- Oath of Allegiance

- Affidavit of Election of Philippine Citizenship

APPLICANT WHO HAS BEEN NATURALIZED

- Identification Certificate of Naturalization

- Oath of Allegiance

PASSPORT - FOR MINOR APPLICANTS (BELOW 18 YEARS OLD)

General Requirements:

- Confirmed Appointment (except for 1 year old and below)

- Personal Appearance of the Minor Applicant

- Personal Appearance of either parent and valid passport of parents (if minor is legitimate child)

- Personal Appearance of mother and proper ID or valid passport of mother (if minor is as
illegitimate child)

- Original Birth Certificate of minor

- Document of identity with photo, If minor is 8-17 years old, such as school ID or Form 137 with
readable dry seal

- Marriage Certificate of minor's parents duly authenticated by PSA (for legitimate child)

- Original and Photocopy of valid passport of the person traveling with the minor

PASSPORT - IF BOTH PARENTS ARE BASED ON ABROAD

Additional Requirements:

• if minor is not traveling with either parent or alone

- Personal Appearance

of either parent
- Affidavit of Support and Consent (ASC) executed by either parent indicating the name of the
traveling companion and relationship to the minor.

- Original and Photocopy of DSWD Clearance

- There is no need to secure a DSWD Clearance if the minor traveling abroad has a parent who are in
Foreign Service or living abroad or are immigrants.

• If both parents are based abroad:

- Affidavit of Support and Consent

- Special Power of Attorney (SPA) with an attached Photocopy of either parents valid passport

- Original and Photocopy of DSWD Clearance

• If Minor is legitimated by subsequent marriage of parent:

- Authenticated BC from PSA must include annotation regarding new status as legitimated and the
full name of the child

• If minor is illegitimate but acknowledged by father:

- BC from PSA reflecting surname of father with Affidavit of Acknowledgement and Consent to use
the surname of the father

• FOUNDING

- Certificate of founding authenticated by PSA

- DSWD Clearance

- Passport of the person who found the applicant.

- Letter of authority or endorsement from DSWD for issuance of passport.

• Orphaned minor applicant

- Authenticated Death Certificates of parents from PSA

- Court order awarding guardianship of the orphaned minor applicant or substitute parental
authority under Article 214 & 216 of the Family Code

PASSPORT RENEWAL
• General Requirements

- Personal Appearance

- Confirmed Appointment

- For NCR-based applicants, present a printed application form downloaded from DFA Appointment
System

- Most recent or expired passport

- Valid picture ID's and supporting documents to prove identity.

• Additional Requirements

Brown passport or passports issued prior to 01 May 1995

- Bring original brown passport and photocopy of passport pages 1,2,3,4 and last page

- Birth Certificate in Security Paper issued by PSA or CTC of BC

• Green Passport or passports issued after 01 May 1995:

- Bring original green passport and photocopy of data page and last page of passport

• Machine Readable Passport (MRP) issued in the year 2007:

- Bring original passport and photocopy of data page and last page of passport

- Birth Certificate issued by PSA or CTC of BC

• Electronic Passport ( E- Passport)

- Bring original e-passport and photocopy of page 2 (data page)

• For women who would change their civil status from single to married:

- Marriage Contract in Security Paper (SECPA) issued by PSA or CTC of MC

- Report Marriage duly authenticated by PSA if married abroad

- Original and photocopy of Commission of Filipino Overseas, Guidance and Counseling Certificate of
Attendance

• For married women who would like to revert to their maiden name

- Birth Certificate or Certified True Copy of BC


• If already widowed: Death Certificate in Security Paper (SECPA) issued by PSA of the deceased
husband

• If marriage is annulled: Annotated PSA Marriage Contract and CTC of Court Order effecting
annulment.

• If divorced: CTC of Divorce Decree duly authenticated by the Philippine Embassy

LOST PASSPORT

• General Requirements

- Personal Appearance

-Confirmed Appointment for applicants with expired passports and issued travel documents

- For NCR-based applicants, present printed application form downloaded from DFA Appointment
System

- Requirements for the first-time passport applicants are still prescribed.

• Additional Requirements

In case passport was lost and cannot be found

- Submit Notarized Affidavit of Loss ( with detailed explanation on when, where and how passport
got lost)

- If passport is still valid, submit Police Report

- Photocopy of the first page of lost passport (if available)

• In case passport was already declared lost by applicant but eventually found within 15 day
clearing period:

- Submit Notarized Affidavit of Retrieval (with detailed explanation)

- Original and photocopy of data page and last page of the passport

•In case mutilated or damaged passports:

- Submit Notarized Affidavit of Mutilation ( with detailed explanation)


- Original and photocopy of first and last page of mutilated or damaged passport

• In case applicant was issued a Travel Document:

- Original Travel Document

- Submit Notarized Affidavit of Explanation Regarding Issuance of Travel Document ( with detailed
explanation)

- Original and Photocopy of data page and last page of cancelled passport (if passport is still in
possession of the applicant)

VISA APPLICATION
- An Endorsement made on a passport by the proper authorities denoting that it has been examine
and that the bearer may proceed.

- A signiture of formal approval by a supperior upon a document.

Abandoned Minor Applicant

•Court order awarding guardianship of the abandoned minor applicant or substitute parental

•Original and Certified True Copy (CTC) of the PSA amended Birth Certificate

•Certified True Copy (CTC) of the Court Decision or Order on Adoption and Certificate of Finality

•DSWD Clearance for minor applicant, if travelling with the person other than the adopting parents.

In case The Applicant is For Adoption by Foreign Parents

•Certified True Copy of the Decree of Abandonment of Child

•PSA Death Certificate of the child's parents or the Deed of Voluntary Commitment Executed after
the birth of the child

•Endorsement of Child to the inter-country Adoption Board by the DSWS

•Authenticated Birth or Foundling Certificate

Minor Applicant Whose Parent or Annuled/Divorce

•Court Order Awarding Guardianship of the minor applicant or substitute parental authority

•DSWD Travel Clearance

•PSA Marriage Certificate with annotation or Nullity or Annulement Decree

Minor Applicant whose Mother is Likewise a Minor

•Personal Appearance of mother and maternal grandparent/s

•PSA Birth Certificate of minor applicant and mother

•Affidavit of Support and Consent executed by the maternal grandparent/s indicating the name of
the travelling companion

•DSWD Clearance if minor will be travelling with the person other than the maternal grandparent/s

•Proof of identity of mother and maternal grandparent/s (Please refer to List of Acceptable IDs)

5.7. General Rules of Ticketing

Briefing before Ticket Release:

Once the ticket has been completed, the following steps should be followed when handling over
the ticket to the passenger
1. Review the flight itinerary with the passenger.

2. Inform the passenger of the restrictions of the ticket, if any, such as validity, rebooking
penalties, endorsable, etc.

3. Advise the passenger of his free baggage allowance for checked in baggage, as well as carry-
on baggage.

4. Advise the check in time, departure time and departure terminal.

5. Recap the seat number that has been confirmed, as well as any special meal requested. 6.
Check the passenger's required documents, if any, and provide additional pertinent information
if necessary.

7. Remind the passenger of the terminal fee that has to be paid after check in.

8. Remind the passenger to keep all his boarding cards and copy of electronic ticket in case his
air miles are not credited to his account.

9. Remind that passenger that reconfirmation is usually not required but it is best to always
reconfirm onward/return bookings.

10. Wish the passenger a pleasant trip!

Issuance / Release of Ticket:

1. Each Passenger including an infant must be issued a separate ticket.


2. Ensure that the passport/ld name matches the name indicated on the ticket.
3. Ensure that your PNR has been finalized and flight details same as in ticket

- Must be counter signed in the presence of the ticketing agent or cashier
- Compare the signature with the Passport/identification of the owner of travelers checks.

Procedure to follow when accepting credit cards:

1. Verify that credit card is signed.

2. Check the validity/ expiry of credit card.

3. Compare the signature the cardholder with the passport

4. Obtain an approval code from credit card company before releasing ticket.

5. Verify card number on charge slip.
