WHEREAS, CHED and DepEd are the authorized agencies to allow the conduct
of face-to-face classes, Priq to approval. schools are required to obtain the local
government's concurrerrce;
WHEREAS, under Section 455 (bxlxvii) of the tocal Government Code the
Mayor may carry out such emergency measures as may be necessary during times of
calamity;

Task Force on Solid Waste Management
At any time, the Gty Mayor may, at her discretion, change the composition of the
Quezon City Quezon City School Sa{ety Inspection and Certification Team for it be
more responsive to the goals of this Executive Order.
4. FUNCTIONS. The Quezon city School Safety Inspection and Certilication Team
shall fulfill the fo[owing functions and responsibilities:

4.3 Engage h the iaforrnation and advocacy campaigns on the safe conduct of
face-to-face classes during the pandemic;
4,4 lncorporate with Helpline 122, complaints agairut schools which do not
comply with the minimum public health standards;
4.5 Veiify eligibility or compliaace of schools for Salety Seat Certificatio4 and
4.5 Conduct inspectioru of schools, both physical and virtual (tfuough video and
submission of Erateria.ls).
5. CITIITIFICATTON PROCEDURE.
5.1 Application.
5.4 Renewal. The SaIety Seal shall be valid for a period of six (6) months and
subiect to renewal one (1) month prior to its exptation.
5.5 Warranty. Display of the SaIety Seal strall constitute a waranty by the
institution corrcern oI continuing compliaice with the minimum standards
and guidelines under the Safety Seal Program. Institutions found to have
mi$epresented t]rcir compliance may be subiect to the apProPriate sanctions
includiag revocation of SaJety Seal until corrective measures are undertaken.

8. FUNDING. Funding requtements shall be sourced and drawn as appropriate
and avaiLable from the respective deparhnents involved.

safety protocols and measuies needed for reopening oI the school in the event of school
lockdown.
Presence of Sc-hool DRRM Team who shall ensrrre that contact tacing activities as
requi.red by the CESU are initiated, and completed among the possible dose contacts
among DepEd persorurel and learners.
