MABUHAY ELEMENTARY SCHOOL

Project Proposal
School Year: 2022-2024

I. Project Title: Project SUD-AN (Support Undernourished pupils’

Development and Achievement through Nutrition)

II. Project Proponent: Project SUD-AN will be lead, checked, and monitored
by the members of the School Planning Team and School
Project Team assigned under ACCESS together with the
School Principal and other stakeholders.

III. Project Rationale: Current data of the Pupils’ Health and Nutrition Status
of Mabuhay Elementary School is very alarming. It shows 23
(10.22%) wasted pupils from the different grade levels.
To adhere to the mandate of the Department of
Education #28 S.2018 or The Policy Guidelines on OPLAN
KALUSUGAN and to address the issue of malnutrition, the
members of the SPT under ACCESS came up with this
PROJECT SUD-AN in which the primary purpose is to
eradicate malnutrition for the next three years.

IV. Project Objectives: To decrease the number of Wasted and Severely

wasted pupils to 0 from 23 (10.22%)

V. Project Description: The project will be administered by

Mabuhay Elementary School Teaching Force headed by the
School Head, members of the Barangay Health Workers group,
parents and other stakeholders.

VI. Project Duration: The project will be implemented f0r three years,
from the School Year 2022 up to the School Year 2024.

VII. Target Beneficiaries: Wasted and severely wasted pupils

VIII: Proposed Budget : Php 40,500 year 1,

Php 30,500 year 2 and
Php 30,500 year 3
The budget is inclusively for the feeding program, purchase
of vitamins, expenses for the purchase of seedlings for the gulayan
sa panimalay activity and gulayan sa paaralan project.

IX. Monitoring and Evaluation:

Project SUD-AN will be closely monitored by the school

feeding coordinator, school GPP coordinator, school head and the
teachers assigned as SPT members under ACCESS.
