AFFIDAVIT OF ARREST

We, P/Sgt. RICARDO DALISAY AND P/Sgt. ADOR DE LEON, both of legal ages,
and both members of Phillipine National Police (PNP), presently assigned at the
Digos City, Police Station, after having been duly sworn to in accordance with law,
do hereby depose and state that:

1. At around 10:30 in the morning of November 24, 2022, MANGI NGILAD, 22 years
old, Filipino, janitor of 5 Eleven in Quezon St. Barangay Mahayahay, Digos City,
appeared at the Police Station 2 Digos City, to report that there’s stabbing incident at
around 9:55 in the morning of the same day at 5 Eleven located at the Quezon
Street Brgy. Mahayahay, Digos City.

2. After receiving the report, we immediately proceed in the located incident, we saw
the person Angel F. Perez, 29 years old, Married, employee of 5 Eleven, and a
residents of Purok. Sampalok Digos City.

3. That upon arriving at the place of the incident, there’s a volunteer witness, a
certain ANNE J. CRUZ, co-worker of the victim, informed us that MARIO
ESPINOZA, 31 years old, Filipino, Tricycle Driver, Married residents of Purok.
Sampalok Digos City, stabbed Angel F. Perez 3 times at the leg portion using Boning
Knife that may cause the death of the victim.

4. Furthermore, Anne J. Cruz informed us that the suspect immediately escaped

after the incident happen, carrying his boning knife and thereafter boarded on a
Tricycle with a plate number ABC 2345 as a getaway vehicle;

5. We immediately informed our Office/Unit confirming the incident. Shortly

thereafter, in coordination with Sulop Police Station, and together with Anne J.
Cruz, we conducted a hot pursuit operation in the residence of the suspects at
Purok. Sampalok Digos City;
 6. Upon arrival thereat, MARIO ESPINOZA we arrested upon the positive
identification made by the witness Anne J. Cruz.

7. We confiscated the 1 Boning Knife use to commit a crime and Tricycle boarded by
the fleeing suspect with a plate number ABC 2345.

8. We thereafter brought the suspect to Police Station 2 Digos City for further
investigation and proper disposition.

9. We execute this affidavit to attest to the truth of the aforementioned

facts and to support the filing of appropriate case in court against the
arrested suspects.
