1. On December 02, 2022at about 4:35 pm a shooting incident in brgy. sapang Jaen
Nueva Ecija, victimizing one Heraldo Punsalan 42 year’s old, married and resident at
Brgy. Sapang Jaen Nueva Ecija . The suspect identified as Alejadro Fernandez, 34
year’s old, resident Brgy.Dampulan Jaen Nueva Ecija.
2. Initial investigator the victim was identified as Heraldo Punsalan according to
witnesses a parked SUV was suddenly shot and with the help of the police who were
on stand by at the check point is that the suspect identified as Alejandro Fernandez
was found and a caliber 45 was found which is suspected to be the one used to kill
the victim and those who saw him said that the victim only bought food at a 7/11
and entered his car, he was suddenly shot, five bullets hit the victim,
3. the victim Heraldo Punsalan was immediately rushed to the hospital but was
declared dead on arrival.

SCENARIO
On December 12 2022 at around 3:00 PM, joint operation led by PCI RICARDO CAPERAL of PIB,
NEPPO, PPSC NEPPO and team of this station led by PSI JARRED STA ANA ACOP Implemented
the Search Warrant No. 08-17 duly issued on DECEMBER 2 2022 by HON. CELS0 O. BAGUIo,
Executive Judge, RTC, Third Judicial Region,Jaen, Nueva Ecija, for Violation of REPUBLIC ACT NO.
7659 Art. 123 (Murder) against ALEJANDRO FERNANDEZ 34 years old of Brgy.Dampulan Jaen ,,
Nueva Ecija Upon arrival. In the presence of Brgy l captain Ronnie Matias
On December 2 2022 a shooting incident occurred in Barangay sapang JAEN Nueva Ecija and
the victim was identified as Heraldo Punsalan according to witnesses a parked SUV was
suddenly shot and with the help of the police who were on stand by at the check point is that
the suspect identified as Alejandro Fernandez was found and a caliber 45 was found which is
suspected to be the one used to kill the victim and those who saw him said that the victim only
bought food at a 7/11 and entered his car, he was suddenly shot, five bullets hit the victim, the
victim was immediately rushed to the hospital but was declared dead on arrival
Cases for the crime of Violation of Section 04, Art. 123 of RA. 7659 (shall be deemed guilty of
Murder and be punished by reclusion temporal.”) Prepared for filing against arrested suspect

AFFIDAVIT OF WITNESS
I John Eric Cruz, 23 years old of age single resident of Sapang Jaen Nueva Ecija, having
sworm in accordance with law, do hereby despose and state.
1. That I was present and personally saw that accused identified, committed the crime of
shooting against the victim Heraldo Punsalan.
2. I’m inside of the 7/11
3. That I was the one who see the suspect shoot Heraldo Punsalan .
4. That I tried to scream to tell him that the suspect was going to shoot him but its to late
5. That I saw the suspect took gun from his pocket and shoot Heraldo Punsalan five times .
I executed this affidavits to atleast the truthfulness of the foregoing fact and support the filling
of criminal cases against the suspect for shooting incident.
