AFFIDAVIT OF GUARDIANSHIP

WE, ANALYN ERADIL CUARTERON and VILGIE GARDIGO

LANQUENO, JR., Filipinos, both of legal ages, and residents of Prk. 1, Baras, San
Miguel, Surigao del Sur, after having been duly sworn to in accordance with law,
depose and state:

1. That we are the parents of DAVE CUARTERON LANQUENO who

was born on May 09, 1997 at P-3, Magroyong, San Miguel, Surigao del Sur;

2. That since birth until present, our son is under our care and custody;

3. That we are competent to receive in behalf of our son any amount due him;
and

4. That we are not imbeciles, insane, vicious persons, habitual drunkards or

habitual criminals, and have not abandoned, neglected or refused to support our son
to commit offenses against the law;

5. That we are executing this affidavit to attest to the truth of all the foregoing
statements and for whatever legal purposes this may serve including the application
of our son in the Armed Forces of the Philippines.
