We would like to inform your good office that we have done several follow-ups to the
contractor of your company for the payment of the Nutri Guide Book. The check was dated
December 12, 2018 amounting to Php 12,522.89. We have received no response from the
said contractor after several follow-up attempts.

In view of the above, we would like to know if you are still interested to claim the check or
we will set up the stale check for write off. Your immediate response will be appreciated.
