District I, Babatngon, Leyte, perpetrated by her stepfather JOSE ESWAN AMPATIN.

Further, request the client to undergo Laboratory Tests available at your good health unit
such as Fecalysis, Urinalysis, Chest X-ray, Blood Type, and Pregnancy Test. Kindly, request also to
furnish this office Medical Certificate as one of the admission requirements of the DSWD Home for
Girls.

victim of sexually abused perpetrated by her stepfather, Jose Eswan Ampatin.

Her mother, Maribeth Agoy has decided to admit her daughter to DSWD Home for Girls in
Palo, Leyte for her safety and security as the alleged perpetrator continuously find and threatened
to kidnap and kill her daughter.

Attached in this letter is the list of Admission Requirements of the DSWD Home for Girls for
your reference.
