1. What action to be taken on this scenario and how to prevent accidents on board ships?

Every crew must follow the instruction of their head to ensure the safety. Personal
safety on board ships is to be able to recognize what is wrong, and how and why. By giving the
most basic appreciation of hazards then allows them to appreciate the actions they need to take
to protect themselves, and to safeguard others. Routine maintenance, inspections and testing of
firefighting and life-saving appliances, including drills and exercises for enhancing crew training
in their use, should be carried out effectively.

2. Discuss the possible cause and effect about the problem?

The cause is the engine crew was carrying out a renewal of lube oil filters, while engine officers
were participating in a safety meeting conducted at the engine control room. As a result, engine
ratings had been carrying out their tasks without the supervision of engineer officers. The lack of
knowledge or inadequate training, incomplete understanding of ships and ship operations. 

The effect in a few minutes, the engine room had been covered with black smoke and it was
difficult for the crew to breathe or see clearly, thus they evacuated machinery space
immediately. It can be tragic and can often cause a massive loss of money and lives. These
events can result in injury, property damage and even death.

3. Give the corrective/ Preventive action when emergency occur

Exit routes should always be well lit, wide enough to accommodate emergency exits,
clear of debris, and should keep personnel safe from additional hazards. Being a responsible,
trained individuals who can supervise and coordinate activities to ensure a safe and successful
evacuation. The coordinator should be responsible for assessing the situation and determining
what actions should be taken. Must be activate and oversee the emergency procedures and
notify and coordinate with outside emergency services. Having enough space to comfortably
and safely accommodate all people.

4. What are the learnings and information that you can apply in real situation?

Knowing what to do in an emergency is just as important as knowing how to prevent


them from happening in the first place. Keeping calm and knowing how to respond to an
emergency may save your life or those around you. When an emergency occurs, the priority is
always life safety. Knowledge from these incidents allows for comparison with the
own situation and systems and enables you to generate creative solutions and take time to
prioritize the measures. Responding to disasters can be both rewarding and stressful. Knowing
that you have stress and coping with it as you respond will help you stay well, and this will allow
you to keep helping those who are affected.
