AFFIDAVIT OF EMPLOYMENT

I, KELVIN TIMOTHY CASTILLO VALINO, Filipino, of legal age, single, and a

resident of Lot 2, Block 13, Park Place Avenue, Park Place Executive Village, Barangay
San Isidro, Municipality of Cainta, Province of Rizal, under oath, do hereby depose and
state:

That, I was a former employee of MAKATI SHANGRI-LA, located at Ayala

Avenue, Corner Makati Avenue, Makati City, as a Banquet Waiter, from July 10, 2012 to
December 10, 2012. I intended to secure a Certificate of Employment from the said
company, it so happened, however, the same was already closed and ceased to operate,
thus, I cannot secure the said certificate. Attached herewith is a copy of my Pay slip as
Annex “A” issued by the said company as a competent proof that I had been employed
with the same;

That, I am also a former employee of ASTORIA PLAZA FULL SERVICE

RESIDENTIAL SUITES, located at No. 15 J. Escriva Drive, Ortigas Business District,
Pasig City with the position of F&B Attendant and Barman from December 13, 2011 to
May 13, 2012. I applied for the issuance of my Certificate of Employment but the same
was yet to be issued;

That, I was also a former employee of PHARMALINK A DIVISION OF

ZUELLING, located in Taguig City, with the position of Medical Promotions Specialist
from July 24, 2017 to October 6, 2018. I applied for the issuance of my Certificate of
Employment but the same was yet to be issued;

I am executing this Affidavit to attest to the truth of the statements and for
purposes not contrary to law, morals, good custom and public policy.
