AFFIDAVIT OF DISCREPANCY

I, ADORA M. DANAC-DAHONAN, Filipino, of legal age, married and a

resident of Gruar Sto. Domingo, Municipality of Cainta, Province of Rizal, under
oath, do hereby depose and state, that:

I was born of Anita B. Marquez and Juanito D. Danak on May 29, 1977 in
Cainta, Rizal, where I was named ADORA M. DANAK. My birth was reported to
the Local Civil Registrar of Cainta, Rizal. A copy of the said Certificate of Live
Birth under Registry No. 534 is hereto attached as Annex “A” and made an
integral part hereof;

Since childhood, I was made to believe that my birth was not reported and
register since the then National Statistics Office (NSO) issued as Negative Result
certifying that no record of my birth can be found;

Basing on such fact, sometime 1997, I caused that “late/delayed

registration of my birth. In the subsequent Certificate of Live Birth, my name
registered therein is ADORA M. DANAC. A copy of the belatedly registered
Certificate of Live Birth under Registry No. 97-3252 is hereto attached as Annex
“B” and made an integral part hereof;

It so happened, however, when I requested a copy of my Certificate of

Live Birth at the Philippine Statistics Authority (PSA), the Certificate of Live Birth
under Registry No. 534 which has the name ADORA M. DANAK registered
therein;

By virtue of this affidavit, I hereby attest that ADORA M. DANAK and

ADORA M. DANAC refer to one and the same person, and that I intending to
use the maiden surname DANAC;

This affidavit is being executed to attest to the veracity of the foregoing

statements for purposes not contrary to law, morals, good custom and public
policy.
