Description
On 09.02.2022 in morning after daily tool box talk, Mr. Praveen Karambele (Site Foreman) has appointed group of 4
workmen for steel cleaning work at mill feed bin area, under the supervision of Mr. Babuli Behra (Site In-charge).

Among the appointed crew, one of the workmen naming Mr. Alex Luogon (Helper); while ascending the ladder have
slipped from the third rung (From ground level) of ladder and due to such sudden slip he have jumped on ground. Same
has been witnessed by the site foreman Mr. Praveen Karambele and reported the near miss to the site HSE team, on
which corrective action has been taken & implemented on site.
