INCIDENT REPORT

Details of incident

This morning a new arrived scholar was manipulating a flask full of acid, he was advised to
wear the basic protection material, an apron, a pair of googles, and a mask, etc. The scholar
started the chemistry practice without googles and when he began to introduce the acid on
the flask the acid steam arrived to one of his eyes. Immediately he was taken to the water
googles in order to clean all acid from his eye’s cornea. Ten minutes later an ambulance
arrived and then he was taken to the hospital for a more exhaustive evaluation by a doctor.
Currently he is stable and recovering from his injuries in Verge dels LLiris Hospital.

The scholar’s name is Lewis Palm, his in his first week of practice in our institution.

Assessment

Mr Lewis Palm was manipulating an acid sample in the lab. He wore the apron, a mask, but he
didn’t wear his googles, because there weren’t enough googles for all students, so he decided
to manipulate the acid flask without googles.

Action taken
In order to prevent more incidents like this one, the rector has ordered to buy extra protection
material to ensure, all teachers, scholars and students have security materials at their disposal.
Also the scholar has recognized his fault in the incident, justifying the fact that although he was
advised, he ignored the advices of the teacher in charge.
