1. EMERGING FAMILY- Couple strives for independence from their parents
and to develop a sense of responsibility for family life.
2. CRYSTALLIZING FAMILY- To assume responsibility for growth and
development of individual members and outside organizations.
3. INTERACTING FAMILY- Assumption of responsibility for “continued
survival and enhancement of the nation.”
4. ACTUALIZING FAMILY- Assume the responsibility for sharing the wisdom
of age, reviewing life and putting affairs in order.

affiliation.
2. B – Compile collected data into a database.
3. ABE - Birth statistics are an example of secondary data the nurse
should review. Previous health survey results are an example of secondary
data the nurse should review. Health records are an example of secondary
data the nurse should review
4. A - Informant interviews are direct conversations with individual
community members for the purpose of obtaining ideas and opinions.
5. D - The nurse should include a comparison of program impact to similar
programs as part of the evaluation plan. This comparison assists with
determining the efficiency of the program.

WRAP-UP ACTIVITY
Family health assessment is the process of gathering information regarding
health promotion and illness preventive initiatives from the family. The
nurse's judgments regarding the family composition, norms, standards,
theoretical knowledge, and communication abilities are all part of the
family evaluation. Reviewing existing information, meeting with the
family, interviewing children and youth as appropriate, meeting with staff
from other agencies, obtaining specialized assessments, identifying the
family's needs and circumstances that contribute to the need for child
welfare intervention, making judgments and decisions about services,
documenting information and decision-making with the family are all
important parts of the process, and assessing progress and requirements on
a continuous basis, and sharing information to the family and other
providers in order to establish and update the service plan.
