PROVISIONAL REPORT
CONDITION OF SODA ASH CARGO OF MV. “SOPHIA Z”
IN HOLD NO.1,3 & 5

In compliance with instruction received from SPICA SERVICES (Indonesia) as local correspondent of
THE UK P&I CLUB and their Member, the Owners of MV. “SOPHIA Z”, the undersigned surveyor did carry
out Draft Survey as well as conducting supervision of cargo discharge operation at Tanjung Priok Port,
Jakarta. We report our findings as follows: -

1.0 Prior to commencement of discharging cargo, we conducted inspection of seals Nos. 1, 3 and 5
manholes jointly with all parties attendance and found all seals were in good condition. However,
the Nos. 2 and 4 hatch and manhole were found was un-sealed due to partly of cargo was
discharged at 1st port in Kohsichang And the all cargoes on top layer (surfaces) were found in
good condition.
2.0 On 02nd January 2020 at 14.10 – 14.40 hours LT, prior to discharging, we conducted joint
inspection with all concerned parties (Consignee Surveyor, Shipper’s Surveyor and P&I Surveyor),
our finding / observation Cargo of Hold No.1 at aft nearest the under of part the acces ladder on
top layer (Surface) of Soda Ash the cargo was found damage (Caked) with dimension (Length x
Width x Thickness) = 1 M x 1 M x 2 CM or weight 1 MT. (photographs attached as below)
3.0 On 02nd January 2020 at 14.40 hours LT, commenced discharge cargo from Cargo Hold Nos. 1, 3
and 5 using ship’s crane.
4.0 On 03rd January 2020 at 10.00 hous LT, during discharging at Cargo Hold No. 3 the P&I Surveyor
found that the cargo was found damage (caked) at Portside Bulwark of hold No.1 with dimension
(Length x Width x Thickness) = 3 M x 1 M x 2 M or weight 2 MT. ( photographs as attached
below)
5.0 On 06th January 2020 at 11.30 hous LT, during discharging at Cargo Hold No. 3 the P&I Surveyor
found that the cargo was found damage (caked) at portside stucking at frame with dimension
(Length x Width x Thickness) = 3 M x 3 M x 3 M or weight 5 MT. ( photographs as attached
below)
6.0 The cargo damage (caked) found are being separated from the sound cargo each in the
mentioned holds and will be collected / removed into to jombo bag subsequently to be weighed
at the later stage.

7.0 As per result of weighing, the
total cargo damaged (caked) are…….MT.
8.0 The estimated cargo discharged as per Shore’s Scale until 06 th January 2020 around 803 trucks
= 27,720.050 MT.
9.0 The estimated balance cargo to discharged until 06th January 2020, around 10,784.950 MT.
10.0 The estimated time completed discharging from MV. SOPHIA Z will be on 08 th January 2020 PM.
