1. SPA 6C (2014) Akuan Berkanun
2. Borang Permohonan Pemeriksaan Kesihatan
Akuan This is the flow who do akuan berkanun at mahkamah seksyen jb only (Jalan
Berkanun Air Molek)
- alamat atas ur own home address, alamat bawah just write JOHOR BAHRU
(pls clarify with the staff again if u really scared write wrongly)
1. Use fb scan to scan QR code (not using MYSEJAHTERA app)
2. Go upstairs go kaunter 3 (tingkat1 kaunter3)
3. Give ur IC and show Surat tawaran
4. Then she will cop, that's it
5. No payment, very fast only

BORANG This is the flow for medical checkup at KK Mahmodiah JB


PEMERIKSAAN
KESIHATAN 1. Book appointment thru http://jknj2.moh.gov.my/qrtca/ beforehand
( reason: routine medical checkup)
2. Go kaunter A tell ur reason, u will be instructed to kaunter 9
3. Kaunter 9- get ur number card, wait for ur turn, fill in the details in kad
hosp(besar and kecil), the nurse will give a booklet for saringan kesihatan (she
will tell until where u should fill up), get height, weight, visual acuity and color
blindness check then go kaunter A
4. Kaunter A- get ur number, show Surat tawaran, go kaunter B.
5. Kaunter B- wait for number u get at A, show ur Surat tawaran, no payment,
go kaunter D.
6. Kaunter D- put kad besar on table wait ur turn and get a number & a
sampling cup for urine collection, put that cup at the tray in the tandas
makmal, wait for getting back ur kad besar with result, go kaunter k
7. Kaunter K- put ur kad kecil, wait for the call, get BP, PR check, go to the bilik
pemeriksaan u assign to (eg. Bilik15)
8. Bilik pemeriksaan- wait ur turn and get everything done.
9. Go back to kaunter 9- pass the kad besar and booklet to the nurse, she will
cop "kk cop" beside both examining Dr and saksi punya cop there. Ask the

nurse to help u get ANOTHER MO to saksi.


10. Go kaunter D to get cop makmal as well

Extra notes: u can ask the nurse at kaunter 9 to cop "salinan diakui sah" above
MO sign (as I get my Surat tawaran, salinan IC and salinan ijazah certified by
examining MO for MMC one).

***SAKSI AND EXAMINING MO MUST BE DIFFERENT FOR BORANG***

MMC Check ur mmc no to do rubber stamp


- google “registered dr”, key in ur name and university, other leave it and click
search
- ur provisional registration number will be there (aka mmc no sementara)
- then can do the stamp (if u ady know ur hosp stamp format la)

Sijil Perakuan Pendaftaran Sementara


- WAIT them to tell u to when send document to the email given, send ASAP
once they announce (usually 2/52 before PTM OR next day after eho closed)
- prepare all document needed first and certified with the cop salinan diakui
sah and save IN PDF FORM (I certified all these during medical check up by the
examining MO):
1. Salinan Surat Tawaran (only pg 1-3, need to be certified in EVERY SINGLE
PAGE)
2. Salinan Ijazah (no pointer one)
3. Salinan IC
(there are 2 more documents which does not applicable to me)

**REFER TO MMC CERTIFICATION GUIDELINE, the best and most convenient


one is certified by MO during medical checkup as I mention in above section,
susah la if guru sek or dean or other later say tak lengkap or not accepted
- label each attachment nicely
eg. SALINAN SURAT TAWARAN_FULL NAME_IC NO
- send to the email as instructed

- mine was replied LENGKAP within 9 minutes (efficient btul)


- go register emerits on that stated date (key in IC, email and fill in the details
needed, then print out sijil perakuan pendaftaran sementara (ada button
“letter” there)
KWSP This is the flow for KWSP JB IF U NOT YET OPEN AN ACCOUNT BEFORE (if
(yg penting already member then I think u can download penyata from i-akaun ahli kut)
ada nama, ic, 1. Tell the staff open account and print statement
no keahlian) 2. Go to the kiosk (like AirAsia touch screen one)
3. Masuk IC, letak jari kanan
4. Fill in the details, no telefon (tac no sent to u for I-akaun registration)
5. Submit, take IC & statement “ PENERIMAAN PERMOHONAN PENDAFTARAN
SEBAGAI AHLI KWSP” (this is the one u need if u are the new ahli kwsp as we
have nothing show in statement also)
6. Follow the steps in the picture below to activate i-akaun ahli (very easy one
that picture is in the building there, if not just ask the staff)
That's it.. got about 5 kiosk so actually quite fast

Bank - Any bank account that you want gaji masuk


statement - Bring along your IC
(yg penting - Tell the recipient that you want a letter for the confirmation of account for
ada nama sma work
acc no. Yg -The letter should have stated with your NAME AND BANK ACCOUNT NUMBER,
jelas) signed by the bank officer (but some said not necessary to sign by bank staff,
got bank cop that it is enough)
**HSAJB lapor diri checklist wrote “Salinan penyata akaun terkini atau buku
akaun bank“ I suppose u can photostat ur account book first page(??)

LHDN This is the flow for LHDN JB


(yg penting Buka akaun
ada nama, ic, 1. Register via edaftar, fill up details (can do at home first)
no cukai dlm -Tarikh mula kja: any dates (on registration date also can)
surat) -Nama Majikan: Kementerian Kesihatan Malaysia (I put this) or hosp name
-Other can leave it since there is no asterisk (*)
2. Must screenshot or print it (got sila tekan sni utk cetak utk tujuan
semakan) after successfully registered
- need this screenshot for getting no pin e-filing and Surat pemberitahuan
pendaftaran no cukai pendapatan
3. Wait at least 24 hours (ikut nasib klo cepat process) to 3 days (paling
selamat xya balik2 p lhdn) for that Surat
4. Go kaunter show ur screenshot and ask for Surat pemberitahuan
pendaftaran no cukai pendapatan and no pin e-filing
5. Wait for ur number. Show ur IC and ask for the surat and no pin
6. That's it, take the original Surat and photostat and follow instructions
from e-filing page from step 1 to 2 (step3 to 5 during March when u need to
declare(?) ur income)

Matikan setem
1. Buy from post office better kut because sometimes no setem at lhdn
counter (mine was from an agent(?)idk what happened that time also, was
being instructed to follow that person and pay him) RM30 total for 3 keping
rm10
* If buy beforehand maybe bring ur sticky glue there dunno need to glue by
urself or not
2. Fill up the date on borang perjanjian kontrak with black pen and they will
help to matikan setem (DATE OF PERJANJIAN=DATE OF MATIKAN SETEM)
3. Let the ink dry by putting other paper on top , jgn terus2 simpan, the ink
will smudge especially on the setem one then will kotor2 lor
3. That's all

PTM (24- 1 week before PTM go to EHO PAGE

jb update just try and try again lo, usually after tempat ptm ada sda, then

MCQ just read from question bank 5min blh siap klo ada baca
Overall doc PTM- 4 (all 1 copy except borang profil need 2 with 2 photo )+ 1 copy surat

Lapor diri
-Unit sumber: 12 (10+ senarai semak+ 2 photo) (need 4 photo actually)
-Unit kewangan: sahkan doc 7 (all 3 copies except LHDN 1 copies)
(Extra: need 1 sahkan copy of surat tawaran, surat ptm and bank statement
for tuntutan perjalanan )
-Unit pentadbiran: geran krta+ 2 gmbr (no need photo at all)

** Bring original IC, Ijazah, Sijil Spm

ni tpi kmu - practical and test for adult CPR & infant choking from conscious into

2. Saringan TB (glucometer reading, bp, waist circumference, imbas qr code
then fill up details)

orientation (BTW I am still in orientation, so INCOMPLETE info for orientation session)
