Observation: Electrical panel was observed lifted from GF to roof
top using a shallow material basket. It was also noted that the panel
Lifting Equipment, gears and was not secured during the process of lifting.

that can secure the size of panel.

Observation: An operative was found using a grinder to cut rebar
with improvised handle made of rebar. It was also observed the
overall poor condition of the grinder (using tie-wire to secure the
tightness of the disc guard)

Recommendation: Redco to conduct thorough inspection of all
power tools inside the Redco store. All power tools with missing
parts shall be removed and not to be issue.

3 Unsafe act/condition

Observation: Vertical opening such as ducts, garbage shute, etc
were observed not properly covered.
2 Immediate
Recommendation: Redco to conduct safety walk-through and
identify the openings. Coordinate with concern group to provide
suitable cover and signages.

Observation: Threading machine was operated by non-trained
workers.

HSE Induction/ Recommendation: Redco to strict implement the procedures to the

Observation: repeatedly observed. On-going hotwork activity
without issuing HW permit including STARRT card.

Recommendation: Redco HSEO to monitor by verifying the permit
and control measures of all hotwork activity.

Observation: Mobile scaffold was utilized by 2 operatives without
scaffold tag. Missing top rail also noticed.

Recommendation: Redco to ensure all scaffold structure must be
inspected by competent scaffold inspector.
