Leadership Style Questionnaire

The following items reflect different behaviors by supervisors. Respond to each item
according to the way you would most likely act as the leader of a workgroup. Circle
whether you would most likely behave in the described way: always (A), frequently (F),
occasionally (0), seldom (S), or never (N).

Scoring
1. Circle item numbers 3,5,11,14,17,21,26,28,33,35, and 39.
2. Write an (X) in front of circled items you answered (S) seldom or (N) never.
3. Write an (X) in front of uncircled items you answered (A) always or (F) frequent.
4. Circle the Xs in front of items number: 3, 5, 7, 9, 11, 13, 17, 19, 21, 22, 24, 26, 29,
35, 37, 39, and 40.
5. Count the circled Xs. This is your Employee-Centered score. E_ 8 _

7. Using these two sources, complete your Leadership Style Profile on the
Leadership Style Profile sheet that follows.

horizontal axis below and draw a line up from that point. Next, move to the vertical axis
and mark your Employee-Centered behavior score (E) and draw a line from that point.
The quadrant in which the two lines cross indicates your style of leadership.
