AFFIDAVIT OF ONE OF THE SAME PERSONS

I, Christopher I. Dela Cruz, of legal age, Filipino, and a resident of

#504 Javalde St., Brgy. Sibut, San Jose City, Nueva Ecija, having
been duly sworn in accordance with law, hereby depose and state that:

1. I am the child of Alfonso Dela Cruz and Juanita Dela Cruz;

2. I have used the name CHRISTOPHER I. DELA CRUZ, in all of my

records and dealings such as Government ID’s, school records,
baptismal records, and other documents of similar nature;

3. My Birth Certificate state that my first name is “CRISTOPHER”. It

is the only document which is in my possession to which is my
identity is based as “CRISTOPHER”, when in truth and in fact, the
name “CHRISTOPHER” is the one that I am using since then
instead of “CRISTOPHER”;

4. I am under the impression that my real name is “CHRISTOPHER”

and not “CRISTOPHER”;

5. It was only during at the time that when I secured a copy of my

Birth Certificate from the PSA that I discovered that my name in
the said certificate, and the one that I am actually using for a long
time, has a clerical error, since it is one of the documents that I
needed to secure in order for the necessary permits for the
application of electricity connection to be approved;

6. The name “CHRISTOPHER I. DELA CRUZ” is the name that I am

using since my childhood;

7. I am executing this affidavit to attest that “CHRISTOPHER I. DELA

CRUZ” and “CRISTOPHER I. DELA CRUZ” as appearing in the
copy of my Birth Certificate are ONE AND THE SAME PERSON;

8. I am executing this affidavit to attest the truthfulness and honesty

of the foregoing facts and for all intents and purpose it may serve;

9. This affidavit is not executed in order to evade and contravene any

law, and that I am well aware and knowledgeable of the penalty of
perjury.
