2) Following documents need to be attached with this form:
 Valid CR copy;
 Valid Trade License copy (Ajeer license, Transportation license etc.);
 Valid Zakat certificate;
 VAT Registration Certificate copy, as and when applicable
 IBAN details should be mentioned on supplier letterhead duly attested from the Chamber of
Commerce;
 Signed and stamped Service Contract;
 GOSI certificate (For manpower providers);
 For Manpower providers, a sample of employment contract with their employees;
 Signed and Stamped Promissory Note (applicable for Transportation providers only);
 For Transportation providers, Valid insurance certificate for Land Transit/Truckman’s liability;
