2. Observe the use of double line spacing. However, use only one space between words, after every
comma, semicolon, or colon. Leave two spaces at the end of every sentence before beginning a
new one.
3. The first line of each paragraph and subheading should be indented one-half inch (0.5”).
4. Observe one-inch margins on all sides (top, left, bottom, and right).
5. Set the section heading in uppercase and centered at the top of the page. (e.g., REFERENCES)

i. Title Page
This includes the author’s/authors’ full name/s, the division/s from which the author/s is/are from, the
title of the Action Research, and the date of the completion of the study.
It is suggested that the title of the Action Research be in capital letters and should not exceed 16 words
that can fit in an inverted pyramid form.

ii. ABSTRACT
This should summarize the content of the manuscript (topic introduction, chief purpose/aim,
method, results, and recommendations) with a maximum of 200 to 250 words only.
Topic introduction presents the conditions that propelled the conduct of the research.
The chief purpose/aim states what the study intended to discover
The method mentions how the study was conducted. This includes the design, techniques, instruments
iii. ACKNOWLEDGEMENT
In this section, the researcher expresses his gratitude to the different persons who assisted and guided
him to make his/her action research successful.
In one to two paragraphs, introduce the general area to be addressed by the study. Use the description
to create a sense of interest in the topic.
In three to four paragraphs, reveal the real problems and show why it is urgent to solve them. Use
factual data (obtained by researcher’s observation or quoted from valid references) to support the
urgency of carrying out studies to solve the problem. Relevant results of previous research are also
good to write here.
Include legal basis (e.g. DepEd Issuances) in establishing the foundation of the study.
The last paragraph should bring up a very strong foundation and rationale of the study.
Discussion should be deductive from macro to micro concepts leading to the problem.
Citing some references and sentences from published materials should be limited to up to three
references only.
Specific aspects of the intervention, innovation, or strategy that had been implemented by different
members of the research team should be included in this part of the report with an explanation of the task
assignments.
If there were materials used to implement the innovation, these must also be described here, referring to
them as appendices.
A. Research Design
Provide a brief description of research design suitable to the nature and purpose of the Action Research.
The details should be provided as to who will participate in the research; specific number of people and
the characteristics of those who will participate in the research; and how will the sample be selected
and recruited
If the researcher intends to utilize sample, the sampling design/procedure has to be indicated.
Clear rationale for their inclusion in the study given.
D. Data Analysis
Describe the technique/s for data analysis to be used.
Explain why the technique/s is/are appropriate for the nature of the data/information to be collected and
for addressing the research question/s.
This part of the report presents the outcome of the analysis of research data considering the intervention
implemented.
The following are some of the things that you need to remember in writing about the research findings:
a. Results or findings should be presented in relation to the research question. If there are secondary
questions, then results should be in sequence according to their order. Presenting results according
to the instruments used (e.g. questionnaire items, interview questions, observation points) must be
avoided
b. Results can be presented through tables, graphs, charts, and diagrams, which can be made more
interesting with colors and readable titles.
c. Narrative explanations and interpretation of results should precede tables and graphs.
d. Tables and figures are labeled as “Figure” and presented sequentially using numbers. Labels for
tables are written on top of them, while labels for figures are provided at the bottom.
e. In explaining the results, include implications and cite probable causes of the findings.
f. Discussion of findings should be clear and presented with as much detail as could be extracted from
the analyses.
B. Reflection
One of the unique parts of the Action Research report is the reflection, which follows a narrative format.
In this part, the researcher shares what he or she has learned from the outcome/s of the study.
Noteworthy experiences during the conduct of the study can also be included in this part.
The following guide questions will help you in generating insights for inclusion as reflection.

(1) What have you learned from the outcome of the Action Research?
(2) What have been your learning experience/s as a researcher in conducting the study?
(4) What aspects of the study should be changed or improved in future implementation of the
intervention?
V. Action Plan
Please be guided with the following questions and format in preparing your action plan:
A. What do you plan to do next looking at the findings of your study and comparing them with the
objective/s of your Action Research?
C. What additional evidence might you collect in order to ascertain the usefulness of the intervention?
D. How will you continue to apply the intervention to other students in your class?
E. Based on the results of the study, what can you do to further improve the teaching and learning
process and/or for the innovation, intervention, or strategy to succeed in future applications?
F. What might you do to propagate the implementation of the intervention on other learners in other
classes and/or schools?
G. What plans do you have for the dissemination and utilization of the results of the Action Research?
H. Do the activities displayed on the Action Plan template specify the objectives, activities or
strategies, persons involved, and time frame?
