This document provides a template for an action research proposal submitted to the Department of Education in the National Capital Region of the Philippines. The template includes sections for listing the lead and member researchers, the title of the research, and the division where the research will take place. It provides guidelines for a context and rationale section, where the researchers should introduce the research area, reveal the problem to be studied using data and previous research, and provide a strong foundation for why the study is needed. The template also includes guidelines for a proposed innovation, intervention, and strategy section, where the researchers should describe their proposed solution to the problem based on previous studies and experiences, and explain the rationale, extent, and limitations of the proposed approach.
