I. INTRODUCTION
This Health Safety & Environmental (HSE) Plans to discribe the HSE related program and
activities that will be execute on all work, may be internal or external works of “Kota
Kasablanka” (hereinafter, the Project) which is to be carried out by “Lotte Engineering &
Construction –Totalindo Joint Operation” (hereinafter referred as Contractor). The HSE plan
provides the approach, methods and practices that will be implemented to perform the work in a
safe and secure manner in order to protect and preserve the well-being of personel, property and
equipment. The Site HSE Plan is prepared, based on the Contractor’s HSE management system
and the related standards to pursue Contractor’s HSE Policy. The ultimate goal is to provide the
safe working environment by identifying, eliminating and mitigating the risk encounter while
performing the work and thefore protecting the physical integrity of all person, equipment,
property and environment.
All personnel, including subcontractor’s employees, engaged in the project construction
work is responsible for his own safety and the people around him. This is therefore of paramount
importance that each employee understand all safety rules and standards and particularly those
safety rules and standards specifically concerned with the work the perform.
This HSE plan is intended to be complimentary to the general code of practice and to
the existing atatutory regulation of :
 Government Laws and Regulations.
 Local Municipal Laws and Regulations.
 Labour legislation with regard to medical, first aid, safety and training requirement
 Employer’s rules, regulations and requirements.
 Contractor’s HSE program, procedures, standards and practices.
All contractors / sub-contractor shall adhere to this HSE plan and appoint their HSE Officer
or so acting that must be approved prior to commencement of the work. They are contacted
to perform for LTJO. The HSE plan will be reviewed & revised by the HSE Manager
periodically as and when required /6 monthly review meeting.

II. DEFINITION
For the purposes of this Procedure, the following words shall have the meanings listed Below :

III. PURPOSE
The following document is designed to provide a detailed action plan.The items are
contained safety manual. This safety plan identifies steps that should be taken by LTJO to
achieve the standard of safety requirement based on safety policy statement.

IV. HSE POLICY, OBJECTIVE AND COMMITMENT


The HSE Policy incorporates the commitment of LTJO Management and HSE
management systems and strategies it covers all activities and our staff personnel and sub-
contractors. It is aligned with the relevant HSE codes and standards internationally recognized.
HSE management is committed to providing a safety and healthfull place of work and to
protecting the environment in the location of work. All HSE issue will be considered in all
business and decision making process. The policy will be endorsed by LTJO top management,
and display at all location where the business will be conducted.
To meet this commitment, Contractor’s HSE policy is to promote high standards in
safety performance, heath preservation, environmental care and security through developing
and maintaining an appropriate HSE plan and procedures covering such elements assessment,
education, motivation, participation, implementation and accountability.
A. HSE Policy
This policy statement incorporates the commitment of LTJO Management
Through the implementation of HSE Management system, the LTJO management will :
 Prevent accidents resulting from our activities
 Personnel safety and protection of environment at and around the site for work are
paramount, assess and manage the risk which may arise from our activities for people,
assets or environment.
 All tasks must be planned and performed with concern for the safety and security of
the staff and the protection of the environment.
 Preserve our employees physical integrity and preserve environment
 All employees, including suppliers and subcontractors, are responsible for their own
and their colleagues’ health and safety at work.

 Management must train employees to work safely.


 Full cooperation and liaison must be offered to the Employer, statutory authorities
and local communities.
 Report our performance against HSE objective and target company.
This policy will provide assurance that the applicable legislation, approved codes
of practice and Employer’s requirements will be complied with.
B. Objectives
The followings are the health and safety objectives which all the participants of the
Project shall subscribe and strive to achieve :
 No Fatalities.
 Zero Lost Time Incidents (injuries and work-related illnesses).
 Minimum medical treatments and First Aids cases.
 Continuing reduction of incidents which could lead to any of the above.
 No spills or environmental releases/impacts under the Contractor’s control.
 All wastes disposed of in full compliance with the related laws of Indonesia, the
Employer, and Contractor’s rules and procedures.
 Minimize wastes whenever possible and feasible through adherence to good
engineering and sound environmental practices.
 A strict drug and alcohol policy, LTJO operates a zero tolerance policy for drugs
and alcohol, this is controlled by routine chek carried out of the barrack area
(accomodation area)

VI. DUTIES AND REPONSIBILITES

A. PROJECT MANAGER/ SITE MANAGER

Responsible for all over safety in work of the entire project for ensuring LTJO
supervision and management, comply with LTJO Safety Policy.

B. FIELD SUPERVISORS

LTJO will endeavor to ensure every person who perform work under their reins who can
be affected by OHS Impact are competent persons based on proper education, training or
experience and maintain related work evidence. LTJO also must determine, implement and
maintain procedure to make persons who work under reins aware to HSE impact
responsibility and procedure
Supervisors supervise workers in work directly regarding work procedures, including
safety procedures, at anytime or any where, necessary. Supervisor also check any violations

of safety procedures or unsafe work conditions, and if find the case,foreman will stop work
immediately and take necessary action to correct worker and or facilites before start working
more. Supervisor conduct a daily tool-box meeting for about five minutes regarding work
procedures to be carried out.

C. HSE MANAGER

To maintain a healthy and safe working environment, ensure adequate training be


provided to safety personal for enable team a perform their function on the project and
ensure the safety philosopy and procedure of company are maintained by competent
supervisors and all employees on LTJO KOKAS 3 Construction Project and also provide
report of safety activity to Site Manager & Project Manager.

D. HSE OFFICER
Here are job description of HSE Officer :

1) To conduct effective and efficient patrol of LTJO Construction Area


2) To instruct worker in safe work practices and follow the instructions of assigned safety
coordinator to ensure implementation of safe work procedures and programmes as
directed by safety coordinator.
3) To inspect the work places regularly.
4) To provide a daily report for Safety coordinator and detail of activities performed the
previous day.
5) To report any incident or unsafe condition that be occured.
6) To write safety instruction as requirement and ensure copies of safety instruction are
returned to Safety coordinator.
7) To ensure the workplace is in a safe condition to be prior for commencement of
activities.
8) To stop work on any occasion when work, limb, equipment is in danger condition.
9) To report any stoppage of work to Safety Manager and also to the Construction
Manager before the action has been taken.

E. SECURITY GUARD GATES

2) Check persons and vehicles that entering and leaving through the gates.
3) Check and confirm materials, equipment and tools that be brought in / out the site.
4) Record material passes
5) Interrogate individuals entering or leaving the site.
6) Take over and compile information and report from the previous guards, and submit it
to the security coordinator.

VII. LTJO GENERAL RULES


The project has some established rules, which are therefore the benefit for all. Please
not that stick disciplinary action will be taken against anyone who violates these rules.
1. Staff shall only carry out task for which they have been properly trained and equipped to
do.
2. All time whilst on site staff will wear personal protection equipment (PPE) the minimum
shall be safety helmet, safety glasses & reflective vest and safety shoes. Certain activities
may require additional PPE
3. Horseplay & fighting is strictly forbidden.
4. Running except in an emergency is forbidden
5. Compliance with site rules & safety regulation is mandatory
6. Possesion, use & sale of alcohol & drugs as well as entry to the site under the influence
of alcohol or drugs is forbidden
7. Tampering with safety equipment is forbidden
8. Abosive or aggressive behavior is forbidden
9. Willful damage or misuse of facilities on the job site is forbidden
10. Compliance with security procedures and job site safety is mandatory
11. No site visiting in the night, weekend, or holiday without permission
12. Only author personnel shall of plant equipment
13. Work Time. LTJO start from 07:00-16.00 hours for normal time, 16.00-21.00 hours for
over time and 22.00-08.00 for important work/rest. 6 days works a week.
14. Identification Badge & Vest. All LTJO workers must wear ID badge during they work at
working site, after received the induction training and get induction pass from Safety
department. All worker also will be wear gray Vest with there digit number registration
in the back side and LTJO logo in front left side. On this project LTJO Safety and
Security Dept. will be stricly control to ID Badge and waistcoat use during working time.

“ No Workers Can Going Inside To The Working Area Without ID Badge and wearing
vest ”.
15. Gate entrance.Based on this project the Entrance Gate are been split to:
a. Gate 1, location LTJO construction area, this gate are proposed especially for visitor.
b. Gate2, location LTJO construction area, this gate are proposed for in/out material
vehicle and Heavy Equipment of owner.
c. Gate General, This gate are proposed LTJO employees/subcontractor and visitor as
accessway to entrance construction area.
16. Use of subcontractors. Pricipal subcontractor :
a. Principal subcontractor are those who are engaged on sections of the contract
providing their own plant and equipment and who may also be responsible for
supplying some materials.
All subcontractors will be vetted prior to starting of the project. Subcontractor
agreement will be completed ang signed by each subcontractor to ensure that they are
fully aware of KOKAS 3 PROJECT quality and safety requirements. Principal
subcontractor will generally provide their own site management and site supervision.
However, all their activies will be closely monitored by the dedicated LTJO site
supervisory team.
b. Minor Subcontractors, Labour-Only Subcontractor
These are normally smaller companies or labour-supply agents where safety
awareness will not have been assessed and any acknowledgement of obligation would
be minimal. These include :
 Carpenters
 Steel fixers
 Labour agents
In general employees of these companies will be treated as if they were direct
employees and will be closely supervised by LTJO staff on site.

17. Traffic Route To Site


1. Material Transportation
Transportation of big and heavy construction material on this project should use
main gate (gate general), route of this tranportation can be directly to project

allowed in KOKAS 3 Project is 10 km/hours.


2. Heavy Equipment Traffic
All heavy equipment such as Crane, Dump Truck and Trailer Truck should use same
access route with material transportation.Vehicle Speed that is allowed in KOKAS 3
Project 10 km/hours.

18. Security Rules


a. General Rules
Drugs, Gambling, Alcohol abuse, and fighting is strictly prohibited at LTJO
KOKAS 3 working area and also at mess staff or labour camp.
b. Gate Rules
At all Gates the workers must follow this rules for entering to work site
location. Worker must bring Identification Badge (ID Badge/ ID Card), if they do
not bring, they can not access to going inside working area. If the workers loosed
the ID Card they should report as soon as posible to LTJO HSE Department.
All workers should wear a proper and clean work uniform complete with
personal protective equipment such as:
1. Safety helmet
2. Shoes
3. Hand gloves ( if, necessary )
4. Long pant, it may not short pant

VIII. HSE CONTROL AND MANAGEMENT


A. SAFETY PROMOTION, MEETING AND COMMUNICATION
1. SAFETY COMMITTE MEETING
The purpose Safety Committee meeting is for reviewed safety performance
within a last of three months period and also discuss the steps to improve the future
in order to achievement the Safety policy targets is zero accident fatality, safety
Committee meeting should be attended by members of P2K3 because they are
responsible for the safety performance

2. SAFETY TALKS
Safety Talks, on subjects of site safety in particular and occupational health in
general will be conducted on the LTJO Project and will be held on friday. In
Saturday after safety talk all employee and staff will do general cleaning. To inform
all employee of developments in Safe practices and to convey at an understandable
level, the requirements of the project.

3. MORNING ASSEMBLY / DAILY TOOL BOX MEETING


Supervisor conduct safety tool box meeting before commencement of work.
The meeting consists of work instruction, tools, personal protective equipment
readyness and environment. Guiding for daily working by Supervisor / QC and HSE
Officer.

4. SAFETY CAMPAIGN
Monthly program, this program puspose to increase employee’s knowledge
about accupational safety and health with exiting media.

5. TRAINING
Purpose of the training program to increase the knowledge and awareness of all
workers to safe working processes and safely.
The training program must be tailored to the needs of work in the field so that the
training must be right on target and objectives.
Among the training that needs to be held as follow:

5.2 Working at height
5.3 Fire prevention
5.4 Electrical tool handling “ Bar cutting, Bar banding “
5.5 Confined space work
5.6 Hot work job

6. WEEKLY SAFETY MEETING


Meeting agenda that be hold every once a week, to discuss and find solution
about accupational safety and health problems.

7. SAFETY INDUCTION
For all new employee in the new project, before they start to work they must
follow induction training by Safety Department then they will be given ID Card
( unless the owner and visitors ) when they have passed induction. The content of
induction area:
a) LTJO Safety policy Statement
b) Basic Safety Knowledge
c) Basic Rules of Safety Permit System
d) HSE Guidelines LTJO
e) Hazard Indentification
f) Personnal Protective Equipment

8. VIOLATION
To motivate workers who have worked without accidents or any significant
violation of safety. Prior to the commencement of work on any worksite, LTJO
agree on a safety objective with respect to personnel and equipment, lost time
accident or pollution will be used as the reference for the evaluation of LTJO safety
performance. LTJO employee and sub-contractor will be get punishment as a
violation letter if they do not follow safety regulation.

9. SAFETY INFORMATION BOARD AND SIGN


The Safety promotion will be establish in Safety information board, safety
posters, safety sign board, safety slogan and safety banners. To support the safety
promotion all the information board will be located and installed at prominent
locations in the offices, canteens, workshop and the main entrance to the site and
other conspicuous locations. Safety Department will be provide information and
notification regarding Safety matters and the information and posters are prepared in
both English and indonesia.

In order to encourage employees interest in and enthusiasm for safety, a safety


incentive scheme and promotion should be established.
1. The incentive awards are given to the workers or group for his/their
contribution to safety efforts and performances directly. The recipients of
awards are commended at the Safety committee meeting in accordance with the
safety incentive scheme.
2. Achivement target 5.000.000 ( five million ) hours whitout fatality accident are
given to all workers involved in the LTJO project.

B. SAFETY INSPECTION
1. General Safety Inspection
As part of daily activities and when assigning personnel to work stations. All
supervisors examine area facilities, equipment tools and materials that used by the
employee systematically as: bored machine, circle saw, bar cutting, bar banding,
cutting wheel, welding machine, cable power, lighting lamp etc. Safety department
monitor those same areas as within their normal job scope continually.

2. Periodicall Safety Inspection


Safety officer with PIC of related sections should be conduct periodical inspection
of some equipment as follow:
a. Generator Monthly Inspection
b. Tower crane Monthly Inspection
c. Passanger Hoist Monthly Inspection
d. Scaffolding Weekly Inspection
e. Fire Extinguiser Monthly Inspection
f. Medical Kit Monthly Inspection
g. Bar cutting & Bar banding Monthly Inspection
h. Canteen & base Camp Monthly Inspection

3. Join Weekly Safety Patrol


This agenda is done every once a week. Followed by HSE department of LTJO
and also subcontractor. The activities are concerning about Unsafe condition and
unsafe action in the project.

4. Site Walk Patrol


This agenda done every once a week. Followed by Owner staff, PM / SM,
supervisor, HSE department of LTJO and also subcontractor. The activities are
concerning about Quality, Unsafe condition and unsafe action in the project.

5. Health and Hygiene Inspection


Personnel first-aid Training and suitable first-aid supplies will be furnished by
LTJO at the site. LTJO prepare a sufficient number of first-aid kits in their offices
for slightly injured or sick persons.
LTJO responsible is to have some of their personnel receive standard first-aid
training. LTJO furnish a suitable number of clean and hygiene wash rooms, toilets
and lounges at their offices, camp and Fabrication/Warehouse site. LTJO Office,
Canteens, Workshop, Ware House, always be maintained in a neat, clean and
hygiene condition.

Material Safety Data Sheets (MSDS) of all chemicals and orther harmful
subtances used by LTJO on the site will be made available to the work force and
LTJO safety and related disciplines

IX. SAFE PRACTICE


A. Personal Protective Environment
Personnal Protective Equipment is available for all employees. All employees must
wear clothing suitable for the work they are doing. LTJO will be provided the PPE such
as :

1. Safety helmet
2. Body vest
3. Safety shoes and boot
4. Hand gloves ( if, necessary )
5. Safety belt for work at height above 2 meters
6. Ear protection ( if, necessary )
7. Safety Glasses (if, necessary )

B. Working At Height
All activities and moving will perform at elevated above 2 meter, worker must use
safety belt and safety harness or be provided scaffolding, working platform, handrail and
ladder for access. Safety sign board and barricade area must be provided under the work
area when the working are perform at height. Dropping or throwing anything from an
elevated height are prohibited.

C. Scaffolding Installation And Dismantling


Scaffold system will be used scaffolding frame at all entire location. Scaffolding
system will be erected and dismantling after project completed in accordance with the
manufacturer’s specification, here are:

1. High of upright frame willnot exceed two(2) meters and spacing will not exceed 1.8
meters. Uprightframe must be tied to cross braces and ledger frames.
2. Spacing of ties or stays will not exceed five(5) meters verticaly and 5.5. meters
horizontaly.
3. For erecting and dismantling of scaffolds will be prepared by qualified personnel.
4. Inspection will be conducted by Safety Department for working platform or
scaffoldsand a tag system will be estlabished, and made known to all personnel
concerned.
5. Competent and authorized scaffold inspector will be given assign.
6. At the external Scaffolding LTJO will be covered with Safety Net.
D. Grinding Welding and Cutting
All grinding, welding and cutting equipments will be inspected daily. Defective
apparatus and equipment will be replace or repaired. Combustible material and
equipment such as gas cylinders, rubber hoses and debris will be removed or shielded
from heat, spark and slag from grinding, welding and cutting. Flash back arestor (safety
valve) will be provided at each cylinder bottle of cutting equipment.

E. Rigging And Material Handling


1. Only an experienced and certivied rigger personnel are allowed to do the work of
rigging and signaling
2. Rigger must be check the feasibility of lifting tools "sling belt, wire slings, shackles,
chain slings, etc." before use

3. Rigger must be check and ensure the binding has been conducted good and right
4. When lowering or lifting material from/to the ground, suitable devices, such as a
chute, bag or container with a rope, will be used.
5. During such work, a watchman will carefully monitor, and “No Entry” and “Danger
Overhead” signs, and rope off around the work area will be provided.
6. When lifting a load, nobody will enter under the load..
7. During erecting unstable steel structures, or before the structures become stable using
proper permanent framing and fittings, false work such as guy wires or stays or
temporary supports will be provided to prevent their collapse.
8. Temporary access ladders for climbing columns, hanging personnel cage for
fastening bolts and nuts or welding, and pieces for installing life lines will be studied
and provided before installation of steel structures.
F. ELECTRICAL AND INSTRUMENT WORK ON CONTRUCTION AREA
1. Temporary Electrical Supply Plan
Before the commencement of Fabrication work on the site, an overall electricity
supply plan for fabrication including, electricity demand forecast, transformer banks,
allocation of distribution.Boards, distribution cable routing and rating, etc. will be
prepared and reviewed by qualified electrical engineers and fabrication planning
personnel.

2. Electrical workers
Electrical work including installation, testing, inspection and maintenance will only
be executed by competent electrician/workers and supervised by qualified
supervisors. The designated supervisor will directly in
struct foreman and workers, and before starting the work, he will explain the scope of
the work, work location, work procedure, time, schedule, proper personal protective
equipment (PPE), proper equipment and tools to be used, allocation of workers, etc.
Throughly to all workers concern.
3. Insulation
During routine inspections and tests, the insulation of electrical equipment and the
associated items will be checked. Where it is found to be damaged or incomplete the
item will be removed from the service.
All wiring will be insulated throughly and where there is the possibility of physical

damage, the wiring must be further protected either by metal conduits or armoring.
Insulation shall be provided at all locations where it is possible for workers to come
into contact with live equipment.
4. Earthing
Earth cables will be tested for continuity and resistance to ensure they are effective
forthat particular application. Remedial action will be taken where they are found
inadequate or ineffective. Until such remedial action is taken the item of equipment
will be taken out of service.
5. Isolation Procedure
Detailed as part of the Permit-To-Work system, isolations are intended to prevent
accidental contact with live electrical equipment.
Work shall officially be carried out on electrical systems until a competent or
authorized person has indicated, in writing, on a work permit that he has isolated the
system by means of locks using the lock out/tag out procedure. The person issuing the
work permit must witness the isolation.
6. Work in the Vicinity of Live Equipment
All electrical systems shall be signed with appropriate warning signs. Work adjacent
to electrical systems shall be subject to work permit procedures even where
surrounding areas do not come under such procedures.

G. PREVENTION OF ACCIDENTS DUE TO FALLING OBJECT


1. Barricades
a) Working under elevated locations where other workers are working at heights
will be prohibited, in principle.
b) Guardrails and caution signs, such as “No Entry” and “Danger Overhead” will be
displayed during working at height.
2. Safe Guards / Safety Net
When working under or at elevated locations is unavoidable, safe guards against
falling objects, such as overhead guards, syinthetic fiber nets to protect falling object
or dust etc. Due to perform construction activities at canopy area or at the roof area
also will be furnished to protect the workers at lower levels, safety screen will be
installed on the each table form for the prevention of material falling down and they
will always wear safety helmets

3. House Keeping
Platforms and walkways at elevated locations will be kept clear of any obstacles,
material, tools, etc. which might fall and injure personnel below. These articles will
be bundled or put in cases or bags so as to prevent their falling and injuring persons
or objects underneath their location. Any debris and scraps will be immediately
removed from elevated location.

H. FIRST AID AND EMERGENCY CASE


First aid facilities will be provided by LTJO for small accidents serious/ emergency
accident will be treated at LTJO Clinic and the nearest Hospital.

I. CLEAN UP
LTJO will keep its work area in a neat, clean and safe condition all times and remove
from LTJO premises and vicinity and properly dispose of all debris and rubbish caused
by LTJO Upon completion of work, LTJO will promptly remove all scrape and waste
from companys premises leaving vicinity clean, safe and ready for use.

X. CONSTRUCTION EQUIPMENT AND VEHICLES


A. Pre-Construction Examination and Inspection
Contractor will ensure that, in the early stage of construction work planning,
subcontractors survey the need for construction equipment and vehicles. They should
provide a list of equipment and vehicles needed to safely perform the work. Need for
cranes, hoists, slings, etc, are analyzed considering the load limits as per manufacture's
recommendations.
Each Subcontractor must include in his submitted safety program, a procedure
relating to construction equipment and vehicle inspection, operation, and maintenance
plan. It must be consistent with the manufacturer's recommendations and Employer’s
and Contractor's requirements as contained in the program.
All cranes and lifting equipment shall be throughly examined by an approved
qualified person as applicable by local law and regulations. A report on the result of
examination shall be submitted (in a prescribed form) before equipment is brought into
site.
Operators of all cranes and other heavy equipment must be physically and
mentally fit to operate the assigned equipment. They should be well trained to operate

the particular type of equipment. Operators’ qualification for cranes and heavy
equipment are subject to review by the competent equipment supervisor and safety
manager.
B. Scheduled Inspection and Maintenance
All Subcontractors' construction equipment and vehicles are to be periodically
inspected and maintained according to pre-determined schedule. Complete record of all
inspection and maintenance are to be maintained for Contractor's review and approval.
Construction equipment and vehicles found to be unsafe for operational use shall be
tagged “Out of Service, Do not use” at the operator’s position. Their use shall be
strictly prohibited until they are repaired and certified as fit for use.
C. Equipment Operator Qualification and Training
Contractor and subcontractors shall review the qualifications of all crane
operators, crane maintenance personnel and other mobile equipment operators in
accordance with the following procedure :
(1) Possession of valid driving a license from the government and job site
authorization card signed by Contractor’s HSE Manager.
(2) Successful completion of a practical operating examination administered by
competent and authorized personnel.
(3) Training - Operator responsibilities, familiarity and comprehension concerning all
crane safety and O&M requirements, load capacity charts, rigging methods and
practices, hand signals and other requirements will be examined by competent and
authorized personnel.
D. Guarding and Safety Devices
Contractor’s Site management and supervision shall ensure that all equipment
and vehicles are equipped with guards and that all safety devices required by law and
site regulations are fitted and effective.
E. Construction Equipment and Vehicles Identification
All construction equipment and vehicles will be clearly identified with the
appropriate names and equipment serial numbers.

XI. VEHICULAR AND TRAFFIC SAFETY

A. Driver Qualifications and Training
Contractor’s site management and supervision will ensure that all drivers are
competent and hold a valid license for the class and type of vehicle or equipment driven
or operated. Each driver or operator must have completed the Site Induction Course
before driving or operating vehicles and equipment on site.
B. Traffic Safety Regulations
Contractor’s site management and supervision will monitor and strictly enforce
the following traffic safety rules.
1) Each driver or operator will be given a copy of the site traffic safety regulations
and regular compulsory Tool Box talks.
2) Drivers and operators will fully comply with all traffic safety regulations, traffic
controls and signs.
3) The maximum speed limit on the construction site is 10 km/h.
4) Drivers or operators who blatantly violate the site traffic safety regulations will be
permanently removed from the site.
5) Personnel must not be transported while standing on the bed of trucks or trailers.
6) Personnel will not ride on the body of material handling or earth moving
equipment.

C. Vehicle Maintenance & Inspection


Contractor’s site management and supervision shall ensure that the
subcontractors and their employees will comply with the following requirements :
Vehicles must be frequently inspected and properly maintained. Each vehicle

must be equipped with the required safety equipment in good working order, e.g.
reversing arms, flashing lights, etc.
1) Preventive maintenance will be performed on all vehicles on a monthly schedule.
All safety features will be checked and repaired as necessary to ensure proper
operation.
2) Drivers and operators will perform a pre-operational check of their assigned
vehicles and equipment at the start of each working day.
3) Each driver shall keep a record of the daily and monthly pre-operational vehicle
and equipment checks.
D. Motor Vehicle Accident Investigations
Contractor’s site management and supervision shall give appropriate instructions
and ensure full compliance of the following requirements: All motor vehicle and
equipment accidents shall be reported and investigated. Investigation and causation
analysis is necessary to implement corrective actions to prevent recurrence.
E. Traffic Controls & Signs
Contractor’s site management and supervision will ensure that traffic control and
signs are displayed as required for the site.
The following traffic signs are mandatory as required by the conditions of site :
1) Stop and yield signs
2) Speed limit signs as follows :
Maximum speed limit on site is 10 km/h
3) Pedestrian crossings
4) No parking or standing
5) No vehicle entry
6) No passing or overtaking
7) Low clearance - overhead obstruction or utilities
8) Temporary Traffic Diversions

XII. WORK ENVIRONMENT AND SITE CONDITIONS


A. Fire Prevention and Protection
Contractor and subcontractors will plan the prevention and measures for the
worst possible conditions from the initial stages of construction planning. These

plans will include an emergency evacuation plan. Fire drills and emergency
evacuation exercises will be conducted regularly.
Contractor and subcontractors will implement fire prevention by house
keeping, no smoking in any place, training, meetings, safety patrol, site inspection,
daily work instructions, etc. Contractor and subcontractor supervision will make
workers fully aware of fire prevention policy and strategy through these activities.
1) Fire Fighting Equipment
Contractor’s site management will ensure that adequate and well-maintained
fire extinguishers are provided for each construction site. Contractor’s site
management will also ensure that subcontractors provide an adequate quantity of
well-maintained extinguisher for their premises, equipment and vehicles.
Contractor’s site management will ensure that workmen are trained in the use of
fire extinguishers, and the basic skills of safe fire fighting.
2) Fire Wardens
As part of the overall fire prevention effort, the fire wardens will be appointed
from within the subcontractor work force. Contractor and subcontractors will
jointly implement fire fighting and prevention training and certification of the fire
wardens.
3) Fire and Emergency Simulations
Contractor will arrange simulation opportunities for fire extinguisher training
and site evacuations every six months.
4) Reporting a Fire
Fires will be reported to Contractor’s Site Manager and the Site HSE
Manager. Then, corrective actions can be determined and taken to prevent
recurrence.

B. Occupational Health and Environmental Control


1) Sanitation and Hygiene Measures
In order to provide adequate sanitary conditions for all personnel at site,
Contractor considers the following provisions as a part of the temporary facilities. If

e) Other special hygienic operations
2) Workers’ Rest Space
Contractor considers the following provisions as a part of temporary facilities.
If they are provided by subcontractors, Contractor will indicate guidelines for
planning, installation and maintenance.
 Sunshades
 Smoking huts
3) Noise Exposure
Contractor and subcontractors will take the necessary measures to protect
workmen against the harmful effects of long term exposure to excessive (loud)
noise. Noise exposure will be controlled by reducing sound levels or wearing the
appropriate hearing protection.
4) Illumination
Adequate lighting is essential to ensure safety in work areas, quality work and
productivity. Contractor’s site management and supervisors will make sure that all
work areas and places are properly illuminated.
5) Ventilation
During construction activities, airborne contaminants may be created which
have harmful effects on the respiratory and circulatory system. Workmen must be
protected against these harmful effects through effective controls, i.e. elimination,
isolation or the use of personal protective equipment (PPE).
Contractor’s site management will take the necessary action as practicable and
possible to control and minimize the harmful effects of airborne contaminants such
as dust, fumes, mists, vapors and gases.
6) Environmental Protection
There are generally two categories of pollution and environmental impact that

a construction enterprise needs to be concerned with:


a) Those affecting the environment in the vicinity of the site
b) Those affecting the working conditions within site boundaries and where
control is necessary, considering occupational safety and health
The areas of pollution can be classified into the following four categories :
a) Solid Waste
b) Noise and Odor Nuisance
c) Water Pollution

The following provides to prevent or minimize environmental impact.


Contractor’s site management and subcontractors shall take appropriate measures in
consideration of site conditions and surroundings.
(1) Solid Waste
During the construction, the following solid waste materials will be produced :
a) Steel scrap, such as pieces of steel pipe, structure materials, etc.
b) Concrete and asphalt materials
c) Excess excavated soil
d) Combustible materials, such as timber and veneers for forming work
e) Solid toxic chemicals
These materials must be disposed of properly in such a manner as not to
contaminate or alter the construction site and surrounding environment. It is
prohibited to dump any solid waste in public places or non-designated areas.
It is most important that solid waste materials be segregated according to the
nature of the materials and the method of disposal. Mixing all solid materials will
cause difficulties in disposal and hamper the smooth operation.
Recyclable steel material for disposal shall be handed over to enterprises which
can change these materials into usable form if such enterprises are available in the
country of the construction site.
Material which do not cause any contamination to the surrounding soil
through the process of leaching shall be buried in an excavated pit. Such pits will
be approved and designated by Employer or local authorities.
If materials are combustible and do not produce an adverse influence on
air pollution, they shall be burnt in a furnace. If the materials are combustible but

produce a harmful emission of gases when burnt, it is prohibited to do so.


The disposal of solid chemicals shall be determined carefully by examining the
nature and characteristics of the chemicals.
Solid toxic chemicals shall never be disposed of in any unauthorized place.
The method of disposing of these materials shall be studied and determined at the
beginning of the project, if the production of such chemicals is expected.
(2) Noise and Odor Nuisance
The best way to reduce these nuisances is to eliminate sources from the
working site. However, construction work often produces these nuisances to a
certain extent. If the degree of nuisance may exceed an acceptable level, the
following measures shall be considered in the work planning:
For noise nuisance, workers who are directly involved in the noise producing work
shall wear appropriate noise protection equipment such as ear plugs and ear
mufflers. This will cover people working in the proximity as well. Duration of
such noise producing work shall also be considered. For example, short working
hours are preferable, so work shall be arranged not continuously but intermittently.
For odor nuisance, the work shall be limited to confined or enclosed
buildings. The worker shall wear an appropriate filter type face mask. Odoriferous
materials shall be stored in an enclosed container when not in use. The duration of
exposure to the atmosphere shall be reduced as much as possible.
(3) Water Pollution
The following are sources of water pollution in construction :
a) Sewage water from office and camp area
b) Spillage of oil and grease, and contaminated material from equipment
maintenance shop area
c) Test and flushing water
Due to the concentration of a large workforce in a limited place, the site office,
temporary site facilities and camp are sources of sewage that must be properly
managed.

XIII. ACCIDENT AND EMERGENCY PREPAREDNESS


A. Emergency Procedures
Contractor will ensure that all management staff, supervisors and workmen on

the site fully understand the procedures, and the actions to be taken during
emergency situations. The following will be considered to establish emergency
procedures:
1) Coordinate with the Employer’s representative in establishing emergency
procedures.
2) Make a list of available emergency services and confirm their communication
channels.
3) Ensure that subcontractors clearly understand their responsibility and role in the
site emergency procedures.
4) Designate first aid/medical supplies, and other material and equipment that may
be required for emergency use.
5) Set up emergency response teams.
6) Determine emergency evacuation routes for work areas and the construction
site.
7) Establish a personnel staging area or assembly points for evacuees.
8) Make sure that all personnel clearly understand emergency warning siren
signals or other warning signals used by the Employer, Contractor and/or other
authorities.
9) Provide summarized communication diagrams and necessary maps, and display
them at proper places in the site.
10) Designate a supervisor to conduct a head count and give instructions to
evacuees as necessary.
11) Post emergency information for each work area on the site.
12) Conduct emergency procedure drill periodically.

B. First Aid and Medical Provision


Contractor and subcontractors will provide or make arrangements to provide
adequate first aid facilities, equipment and qualified attendants for work site.
1) Considerations on First Aid
The following are key considerations in the preparation of first aid facilities at
site:
a) Experienced and knowledgeable personnel who can lead the operation and
give necessary training to others shall be assigned. A list of qualified

personnel such as first aiders or nurses shall be provided and published


through all organizations at site.
b) Adequate and properly stocked first aid kits, complying with local labor
laws, shall be kept at locations in accordance with the same laws.
c) An ambulance or other suitable vehicle will be provided to transport the
injured and sick to the nearest medical facility.
2) First Aid Training
Contractor intends to provide training courses so as to increase the number of
knowledgeable personnel and enhance the total site performance in
emergency/first aid cases. In order to realize this intention, Contractor and
subcontractors supervisors will be trained in basic first aid and CPR (Cardio
Pulmonary Resuscitation).
3) First Aid Treatment
First aid facilities shall be readily available to all personnel and at all times at
site. Records of all types and causation of minor injuries and sickness will be
maintained and analyzed by Contractor.
C. Risk Assessment
Risk Assessments are made before starting the work by related Supervisor, and it
will be checked by LTJO HSE Officer base on the step of work and it will be
approved by Project HSE Officer. The project team will carried out the risk
assessment to identify the major HSE risks on the project and provide controls to
eliminating or reducing the risks to acceptable levels. During the development of
the method statement, detailed risk assessment of the specific works will be
carried out. This risk assessment will be attached on work method. Finally, the
Risk Assessment will be produced as the working document that will be briefed to
the team that directly involved in executing the specific works. The main area’s of
risk in the construction of the project are as follows :

XV. PERMIT TO WORK


 Guidelines are included in the Permit to Work form where certain
work/activities will require a permit to work prior to the commencement :

1. Working at height

7. Working adjacent to services

8. Deep excavation

9. Heavy duty lifting operation

And other work requested by EPH. This permit will cover the safe system of
work for operations on existing services. Recommendations are included for a
Permit-to-Work system which can be used as a benchmark for any system
promoted by the client. Prior to the commencement, during work and other
check list/inspection form are also provided.

XVI. SITE INSPECTION, AUDITS, AND PERFORMANCE


MONITORING
1. HSE Inspections
(1) Contractor and Employer teams for periodic inspections will be formed
consisting of the following members:
 Contractor’s Site Manager
 Contractor’s Site HSE Manager
 Employer’s Representative
 Employer’s HSE Manager
(2) A standard HSE inspection checklist will be used to conduct each
inspection.
(3) The Site HSE Manager will prepare a written report on the HSE
problems/deficiencies identified during inspection of each facility and work
area.
(4) When potential deficiencies are found, immediate corrective actions will be
taken for incident/accident prevention.
(5) Reports on the HSE deficiencies will be kept in site office for statistical
purposes and analysis.
(6) Contractor will instruct each subcontractor to submit a weekly report of site
inspection using a standard form.
(7) Crane operator is required to check his crane every day using the prescribed
form.
2. Accident and Near Miss Investigations
1) Investigations
All accidents, incidents and near miss occurrences will be investigated. The
cause or the chain of events that led to the accidents/incidents and near-miss
must be determined. When the cause is known, appropriate actions will be
taken to prevent recurrence. Accident investigations will be conducted
without prejudice. The investigations will focus on finding the facts, not
fault or placing blame.

In the case of a fatality or an accident causing disability, a report (normally


by telephone) shall be issued immediately to the appropriate local
authorities or police. Any accidents will be reported to the Employer and the
Managing Director in head office by facsimile, telephone or e-mail.
Using information from the reports and records, management can determine
weak areas in the HSE plan. These weak areas can then be targeted for
corrective action. Analysis of the data will show the type of corrective
action required. This may be training and education, motivational and
awareness schemes, new equipment and tools or new working methods.
3) Accident Reporting
a. Accident Reporting

The internal reporting procedures to Head Office - Jakarta specified here


are mandatory and additional to any local reporting which may be
required. Accident report will be issued to ensure that all accidents and
incidents occurring on site are identified and reported promptly either to
the Project Manager or to the Construction Manager at site. All such
report will be sent to LTJO Safety Department or Safety Representative
in the Head Office.

The HSE Officer will ensure that all lost time accident, accident
involving third parties and dangerous occurrences are investigated to
determine the basic causes and, what corrective action need to be taken
to prevent re occurrences. All such investigation report when completed
will again be forwarded to the KOKAS 3 Project Manager or Project
Director for review.

b. Immediate Reports

Immediate reports by telephone, if possible, or fax are required in the


following circumstances :

 Fatal Accident ( to own staff or third party )

 Report to local Police

 Telephone report with salient details to Project Manager HSE

The HSE Manager will also prepare a brief report for immediate
distribution to Project manager

4). Hazard Analysis, Unsafe Acts and Corrective Actions


 Site management and supervisors will perform hazard analysis as
required to identify and eliminate the dangers of high risk and
activities.
 Monitor the site, work areas and jobs being performed on a routine
and timely basis.
 Site management and supervisors will take immediate action to
ensure hazards are made safe and unsafe acts corrected.
 Supervisors will give clear instructions for the work to be performed.
Workmen will be made aware of the hazards of their work.
 Adequate and appropriate personal protective equipment will be
provided for the work to be performed.
 As required, workmen will be given training to ensure they have the
knowledge to perform their job safely.
 Communications will be maintained between supervisors and
workmen to ensure effective hazard control and the elimination of
unsafe acts.
 Supervisors will respect and take into consideration the suggestions
and recommendations made by workmen concerning the work
performed. Immediate action will be taken to eliminate reported
hazards and unsafe acts.

5). Performance Measurement


Among a number of diversified measurements available in different countries
and different industries, Contractor uses the following two measurement indices
for assessing safety performance:
 Accident Frequency Rate (FQR)
 Accident Severity Rate (SVR)

XVII. SUBSTANCE ABUSE POLICY


The consumption of any intoxicating substances during working hours or
within such time previous that a person would still be considered intoxicated is
prohibited.
Any such substance such as alcoholic beverages, illegal drugs, inhalants and
prescription drugs are not to be consumed during working hours or within such
time as the person would still be mentally or physically impaired.
The use of prescription drugs during working hours is prohibited unless
verification from the prescription physician states that the medication would not
impair the person’s ability to perform their specific duties. This verification must
be in writing from the physician and on the office letterhead.
Contractor will implement education of the workforce at site safety orientation
courses and safety awareness campaigns of the dangers and consequences of
substance abuse during employment on the site to ensure compliance with the
substance abuse policy.
As standard policy on all security access points and worksite locations,
Contractor’s security staffs are given the power to stop and search person on site,
drivers of motor vehicles are observed carefully. Any person found to be

intoxicated will be immediately refused to access to the site or removed from its
normal duties.
