3.0 Definitions
Valve Team: designated and specific individuals that must all agree prior to operating the well-head
master valve.
Valve Team Designee: a crew member who will be responsible for operating the wellhead and ground

and high pressure area (red Zone) identified and barricaded and/or flagged off.

5.2. A Valve Team will be formed and will consist of the following members:

Mandatory
 Halliburton CT Service Leader
 Halliburton CT Pumping Crew Lead
 Customer Site Representative

pressure, wellhead valve configuration and operation, and priming and testing sequence.
Attendance is to be documented. Communicate wellhead LOTO standards per ST-GL-HAL-HPS-403
and make sure all employees understand valve team role.

5.4. All Valve Team members should communicate that ALL master valve activities are coordinated
through them.

valves that need to be operated.

5.7. Valve Team members shall use a repeat back communication for command verification.

5.8. Valve team designee will operate wellhead or high pressure valves under the supervision of CT
Service Leader. CT Service Leader should have line of sight and radio communication with Valve Team
designee at all times while operating valves.

5.9. Operating districts shall devise a well / manifold valve(s) identification practice that allow for proper
and unambiguous identification during operation.
