SUBJECT: REF. NO. 2020-01-003CONSTRUCTION OF MULTI-PURPOSE

CENTER NO. 3, SUPPORT FACILITIES WITH FURNITURE
AND EQUIPMENT INCLUDING LAND DEVELOPMENT AT
THE BAL-IK PROBINSYA BAGONG PAG-ASA (BP2) DEPOT
ALONG AGHAM ROAD COR. QUEZON AVENUE AND EDSA,
QUEZONCITY

Dear Engr. Zafranco:

Please be informed that your proposal for the above-cited project was favorably
recommendedby the East Sector Regional Office Bids and Awards Committee
(ESROBAC), endorsed by NHA Managementand CONFIRMED by the Board of
Directors as per NHA-BR-6805 dated 28 January 2021 at a contract price of: FORTY
FOUR MILLION NINE HUNDRED SEVENTY SEVEN THOUSAND FIVE HUNDRED
SIX AND 29/100 ONLY (Php 44,977,506.29) to be completed within One Hundred
Twenty (120) Calendar Days pursuant to the 2016 Revised Implementing Rules and
Regulations (RIRR) of Republic Act (RA) 9184, as amended.

You are hereby required to submit within ten (10) calendar days from receipt of this
Notice of Award, the documents indicated below.
Performance security, co-terminus with the final acceptance of the project, in
accordance with any one of the followingschedule:

a. Ten percent (10%) of the contract price —if in the formof Cash or
Cashier's/Manager'sCheck or Bank DraftJGuaranteeor Irrevocable
Letter of Credit issued by a Universal or Commercial bank.

b. Thirty percent (30%) of the contract price —if in the form of Surety
Bond callable upon demand issued by a surety or insurance company
duly certifiedby the Insurance Commission.

The original Official Receipt (OR) evidencing payment of bond premium shall be
submitted to the NHA.

2. All Risk Insurance Policy equivalent 100% of the contract cost, together with
the original receipt of premium payment, which covers:

a) Comprehensive insurance for third party liability due to contractor's direct or

indirect act or omission causing damage to third (3rd) person or persons.
b) Employer's Liability and Workmen's Compensation for all the employees of the
contractors.The contractor may choose to be self-insured provided prior
approval of the Bureau of Labor is secured.
c) Fire, earthquake, flood and such other perils the contractor may deem
necessary for the protection of the construction plants, temporary works and
materials used.

3. A written authorityfrom a senior official of your firm/organization empowered to

sign the contract documents. (If corporation, a Secretary's Certificate shall be
issued by the concerned partyfies)
4. Photocopy of current Community Tax Certificates C and C-1.

5. PERT/CPM and Cash Flow.

6. ConstructionSchedule and S-Curve

7. Manpower Schedule

8. Equipment Utilization Schedule

9. Construction Methods

10. Application for Approval of Construction Safety and Health Program duly
received by the Bureau of Working Conditions of DOLE.

Failure to comply with the above requirements within the prescribed period, shall
constitute sufficient ground for cancellation of the award and forfeiture of the bid
security. This shall be construed as a waiver on your part to accept the award of the
contract withoutfurtherwritten notice, in accordance with provisions of Section 40 of
the 2016 RIRR of RA 9184, as amended.
Further, submission of fraudulent or fa!sified documents and willful misrepresentation
of any informationprovided shall be a ground for suspension or blacklisting in
accordance with Sec. 69.4 of the 2016 RIRR of RA 9184 and the guidelines issued by
the GPPB.
Should you agree with the award of the contract, please sign at the space below for
your conforme and return the Signed Notice to this office within three (3) working days.
