    Brief Description of Accident / Incident / Near Miss: (Attach additional sheet if required):
    A water tanker from M / s. NNS CONSTRCTION used to come inside our site premises to fill the water tank. On
    05.06.2022, the water tanker was coming to our site The driver lost his attention while driving (see down ) for which
    he lost sight of the road, the vehicle got off the road then the driver tried to bring the vehicle back on the road due
    to heavy water load leaning towards canal , he could not bring the vehicle back to road. When the vehicle could not
    come on the road, the driver got out of the vehicle in search of outside help to drag the vehicle on the road in the
    meantime due to the heavy load of water, the vehicle slips further towards the canal and finally fell inside the canal

    Causes of the Accident / Incident / Near Miss:


    1.The driver was absent in mind during driving the vehicle.
    2. Due to narrow road.
    3. Edge protection (Solid) was missing in the canal.
    4.The condition of the road was poor in nature.

    Recommendation / Action Taken for Avoiding Recurrence (Filled By Project Manager/RCM):


    1.The driver must go through training program.
    2.Road shall be widened.
    3.Edge protection or crash barrier shall be provided for the canal.
    4.The condition shall be rectified
