Keputusan Menteri Kesehatan Republik Indonesia Nomor HK.01.07/MENKES/1/2018 tentang Pedoman Nasional Pelayanan
Kedokteran (PNPK) Tata Laksana Thalasemia.
1. Kelasi Besi
• Pemberian terapi kelasi besi dapat mencegah komplikasi kelebihan besi dan menurunkan angka kematian pada pasien
thalassemia.
• Terapi kelasi besi bertujuan untuk detoksifikasi kelebihan besi yaitu mengikat besi yang tidak terikat transferin di plasma dan
mengeluarkan besi dari tubuh.
• Pemberian kelasi besi dimulai bila:
a. kadar feritin serum darah sudah mencapai 1.000 ng/mL; atau
b. saturasi transferin ≥ 70%; atau
c. apabila transfusi sudah diberikan sebanyak 10-20 kali; atau sekitar 3-5 liter.
• Tiga jenis kelasi besi yang saat ini digunakan adalah deferoksamin, defepiron, dan deferasiroks.
2. Bab III Kesimpulan, bahwa thalassemia merupakan penyakit kronik yang memerlukan tata laksana komprehensif. Upaya
diagnosis, tata laksana transfusi, kelasi besi, pemantauan dan manajemen komplikasi, pemantauan tumbuh kembang, dan

50 upaya konseling serta skrining karier merupakan satu kesatuan dalam penatalaksanaan pasien dengan thalassemia mayor.
PNPK ini diharapkan dapat menjadi dasar rekomendasi bagi fasilitas kesehatan primer sampai dengan tersier di Indonesia.
