SUBJECT: PRICE PROPOSAL FOR REBOUND HAMMER (ASTM C805)

Thank you for considering our company to quote on this particular project, we are pleased to
submit our formal offer as follows:

I. SCOPE OF WORKS.

INTEGRAL MATERIALS TESTING CORP. will supply tools & equipment to conduct rebound
hammer test for the above-mentioned project.

II. CHARGES.

1. Mobilization & Demobilization Php. 2,000.00 / Visit

2. Rebound Hammer Test (ASTM C805)

Minimum of (10) location / visit Php. 400.00 / Location
III. TERMS & CONDITION

1. Client shall pinpoint all location to be tested.

2. Client shall assign field representative to coordinate with our technician.
3. Client shall guarantee that the site is ready and free from any obstruction.
4. Client shall provide platform and ladder (if necessary).
5. Client shall facilitate the necessary permit/gate passes needed by our technician.
6. Payment:
 Down Payment: 50% prior to mobilization
 Full Payment upon completion of testing and releasing of official test results
7. Mobilization – One (1) day notification and upon receipt of P.O.
8. Validity of Proposal: thirty (30) days upon receive of the quotation.
