JOINT AFFIDAVIT OF DOMESTIC PARTNERSHIP

We, DON MARK SAPITAN VILLAMER and ERIKA MAE JAVIER ORDIALES, of legal ages,
Filipino citizens and a resident of Block 40, Lot 6, Barangay Fatima II, City of Dasmariñas, Cavite, after
having been duly sworn in accordance with law, hereby deposes and state that:

1. That we are both single and have the legal capacity to contract marriage;

2. That there is no legal impediment whatsoever to our contracting marriage;

3. That neither of us are presently married, as a result of either civil or religious rites to any
person in the Philippines or elsewhere;

4. We are sole domestic partners, and have been domestic partners for at least ___________
(___) year/s preceding the date of this Affidavit;

5. We have been sole domestic partners living together continuously since ____________;

6. That we take responsibility for all our actions and undertakings inside and outside of the
Republic of the Philippines;

7. That we have by these presents mutually agreed, and are in fact now presenting ourselves
voluntarily and of our own free will for whatever legal purposes and intents the same may
serve best.
