ANNOTATION:

For KRA 5 - Objective 19, I have

attached certificates with pictures of my
active participation as I performed various
related works/activities that contribute to
the teaching and learning process beyond
the School/Community Learning Center
(CLC).
As a result, in Indicator 19, I obtained a
rating of 8 implies that I achieved the
objective – Performed various related
