 Supplier- delivered smaller size of eggs, spoiled/overripe banana with residue on the fruit juice and
tampered expiration date.

Actions Taken- informed the Division SBFP Coordinator – Maam Karla Alzate, RN.

BEST PRACTICES:
 Organized SBFP Core group
 Posted in the GC the delivery date and time.
 Assigned teacher will get the NFP and Milk at drop off pint school.
 Checked the expiration date of the NFP before giving to the beneficiaries.
 Test the pasteurized milk before giving to the beneficiaries.

 Delayed in the delivery of Pasteurized Milk
 Some of the pasteurized milk were spilled due to poor packaging.

Action Taken – informed the supplier
