I included an abstract with a compelling opening statement that gives the background of the

proposal. The statement is general and does not require any citation.
I also included a specific problem statement that identifies the gap to be filled.
I also modified purpose statement that exactly matches the manuscript template with the required
elements as stated in the template. The intervention was the AHRQ’s RED toolkit, “Tool 5: How
to Conduct a Post-discharge Follow-up Phone Call” and the outcome is readmission rates. The
population is heart failure patients admitted in a surgical unit of Comprehensive Cardiology
clinic. The duration is 30 days.

that they are the same in chapter two and three.
