gL WORK PROCEDURE HSE UNIT
TITLE: SAFETY UNIT MANAGEMENT
1.0 PURPOSE
‘This procedure provides brief outline of all HSE unit activities being carried out. Main purpose of
this procedure is to perform all the activities according to a well-established documented system,

3.0 RESPONSIBILITY
Unit Manager HSE, Safely Engineer & Staff Employees are responsible to achieve overall
objectives of constant improvement in safe working environment at plant site
4.0 PROCEDURE
4.1 SAFE OPERATIONS COMMITTEE MEETING (SOC)
Itis conducted to review, control and manage continuous improvement in safety performance at the
plant as per safety committees work procedure SAF - WP - IMS - 008. HSE unit prepares safe
operations committee meeting agenda and package. Minutes of meeting are prepared and sent to
all concerned for implementation.
4.2 DEVELOP / REVIEW PROCEDURES
Procedures are developed and reviewed as per requirement
4.3 CONTINUOUS IMPROVEMENT PLAN
According to this plan, safety coordinators (management employees) are nominated to streamline
the safety related activities by their respective departments. Keeping in view the "Employee
Involvement” concept, each unit nominates safety captains amongst staff employees as per work
procedure SAF - WP - IMS - 008
44 INCIDENT INVESTIGATION
4.4.1 Each safely incident is reported to HSE unit on (Form # SAF-1) by concemed
department, ‘
44.2 Incidents are investigated by the nominated employees as per incident investigation work
procedure SAF-WP- IMS-007. HSE unit assists if required,
443 Safety Engineer reviews the incident and prepares Flash Report which is issued after
approval from UM HSE, The nominated team issues detailed investigation report
atterwards.
4.44 Allincidents are categorized and recorded according to their ype & seventy.
45 SAFETY AUDITS
4.5.1 MANAGEMENT SAFETY AUDITS
HSE Unit prepares the Management Safety Audit Schedule and follows for its compliance. The
recommendations are uploaded in Safely Management and Information System which is

‘AUDITS AS PER AUDIT MATRIX
HSE Unit conducts audits as per the approved audit matrix at plant site which includes the audits
such as PSM internal & External Audits, Risk Management and Evaluation Audit, IRR Audit eto.
AUDIT OF UNSAFE CONDITIONS / ACTS:
‘a. Safety personnel monitor unsafe conditions / acts related to plant machinery / equipment /
environment or operations at plant site with focus on special projects.
These observations are discussed within HSE unit and then sent to concerned unit for
rectification
& Serious deficiencies in implementation of Work Procedures are presented in the morning
meeting,
NOISE LEVEL AUDIT
a. HSE unit conducts Noise level audits for the safety of plant personnel as and when required.
SAFETY / FIRE EQUIPMENT AUDIT
a Concemed unit and HSE Unit staff carry out audit of fire extinguishers, safety showers,
SCBA, foam trolleys, fire hydrants and wind socks as per defined frequency and checklist. If
faulty equipment is detected, itis either replaced or repaired and put back in service.
b. HSE unit owns fire trucks equipped with safely & fire fighting equipments. Fire truck operator
conducts their audit via a checklist (Form # SAF- 08),
SAFETY INCENTIVE SCHEME
This scheme has been introduced to increase work safety and maintain injury free environment.
Number of hours worked by all permanent employees including overtime of all staff employees
without lost time injury make safe man-hours. HSE unit maintains its monthly record. Safe million
‘man-hours keep on accumulating until a loss time injury on the job occurs. Next day of its
occurrence, safe man-hours counting starts from zero. JR department sends record of man-hours
(Staff) to HSE unit daily (working days); Management sets a target for an incentive scheme from
time to time according to Work Procedure SAF-WP-IMS-027
SAFETY TRAINING
4.7.4 Practical training of safety / fire fighting equipment is conducted by HSE unit for plant
personnel every year.
4.7.2. Mock fire dril is conducted in different operating areas on weekly basis. Coordination
engineer, fie squad and respective unit personnel participate in the dri. All activities are
‘monitored and recorded via "Fire Dril" checklist (Form # SAF-13).
47.3 Orientation programs are also conducted for new inductees / turnaround manpower and
visitors as per requirement according to the Work Procedure SAF-WP-IMS-028. A
practical demonstration of Safety equipments is given in HSE unit.
47.4 Theoretical and practical trainings are conducted for both Management and Staff
‘employees at plant site as per Annual Training Calendar of HSE Unit.
47.5 Regular training of the staff at HSE Unit is conducted as per monthly UTC (Unit Training
Committee) Calendar and the feedback is sent to TTC.
COMPILATION AND STEWARDSHIP OF RECOMMENDATIONS
HSE unit Is responsible for the compilation and stewardship of the SOC MOM, PHA, PSM, QED,
IRR, MARSH AND PSSR recommendations. The status of these recommendations is presented in
‘SOC on monthly basis. I

HSE unit maintains a comprehensive Emergency Management Plan (SAF- WP- IMS -03) to handle
jan emergency at the plant at any time. This procedure is available in all departments and equally
applicable to all personnel present at the plant.
4.10 PROCUREMENT! STANDARDIZATION OF SAFETY EQUIPMENT
4.10.1 Standardization of equipment is carried out to reduce time between ordering and final
procurement.
4.10.2 Purchase requests are initiated by Safety Engineer & reviewed by Unit Manager HSE as
per budget plan.
4.11 FIRE - FIGHTING FACILITIES
4.41.41 Three Fire trucks are available in HSE unit
4.11.2 Fire fighting accessories ie. Hose pipes, Nozzles, Couplings, Adapters, Valves etc. are
available in HSE unit
4.11.3 All monitoring and measuring devices Le. Oxygen, CO, NOx, Explosivity and Noise
detectors are available in HSE unit.
4.11.4 HSE unit is also equipped with different types of communication system i.e, dedicated
. telephone lines, Hotline and Motorola,
