FINDINGS
Lesion:
There is large infiltrating heterogeneously enhancing solid mass lesion involving right
nasopharynx, fossa of Rosenmullar, toras tubaris and eustachian tube opening on right side
appearing heterogeneously hyperintense signal on the T2W, STIR images and hypointense
signal on T1W images showing restricted diffusion . On postcontrast scan the lesion shows
heterogeneous intense enhancement. The lesion measures approx. 40 x 24 x 33 mm (AP × TR
× CC) .
Local extension:
Anteriorly, the lesion extends into right side posterior choana. No evidence of extension into
right nasal cavity.
Superiorly, No evidence of extension into sphenoid sinus.
Laterally, the lesion involves medial, lateral pterygoid plates and pterygoid muscle with
minimal extension into within the infratemporal fossa. The lesion is also involving right
pterygo-palatine fossa.
Medially, the lesion causes narrowing of nasopharyngeal air way.
Inferiorly, the lesion is involving right lateral wall of oropharynx.
Posteriorly, the lesion is involving right sided clivus and petrous apex. The lesion shows
extension into right eustachina tube and reaching upto right middle ear cavity. The lesion is
also partially encasing petrous segment of right ICA. The lesion shows minimal tethering with
the right prevertebral muscle.
On CT images, there is erosion of medial and lateral pterygoid plates on right side. Erosion of
right sided clivus and petrous apex. Severe bulky osteophyte and degenerative osteoarthritis
involving right atlanto-axial joint.
Lymphnodes:
Few ovoid lymphnodes with maintained fatty hila seen at level Ia, bilateral level Ib and II -
reactive nodes. Largest measures 14 x 6 mm at right level IIb.

The thyroid and the larynx show normal configuration and signal characteristics. Rest of the visualized
muscles are unremarkable. Rest of the visualized bones show normal signal intensity.
IMPRESSION
Large infiltrating heterogeneously enhancing solid mass lesion involving right nasopharynx,
fossa of Rosenmullar, toras tubaris and eustachian tube opening on right side with
extension as described above.
