PATIENT’S NAME MR. ANIL SINGH AGE/SEX 60Y/M
REFERRED BY DR. ULLAS BATRA

CT SCAN: THORAX & ABDOMEN (CONTRAST)


1.5 mm thick slices were obtained to scan entire thorax and abdomen after ingestion of oral contrast after injection of I.V. contrast
medium followed by sagittal and coronal reconstructions.

There is evidence of heterogeneously enhancing soft tissue density lesion with irregular
margins seen at right hilar location encasing right upper lobe, right lower lobe bronchus,
right pulmonary artery with extension of lesion in adjacent right upper, middle and lower
lobes. Lesion measured approximately 4.4 x 4.3 x 4.7 cms. Focal patchy atelectasis seen
involving right upper, middle, lower lobes and left lower lobe. Mild fluid seen in right pleural
space.

Enlarged lymphnodes seen at precarinal, subcarinal locations measured upto 10 mm in


short axis diameter.

Trachea and its bifurcation appears grossly normal.

Vascular wall calcification seen involving aorta and coronary arteries. Rest of the
mediastinal great vessels are normally identified with normal cardiac configuration.

No sizable lesion detected in the region of esophagus.

Bony cage (spine, ribs and sternum) is normal. No evidence of any bone destruction or
injury seen.

Soft tissues of the chest wall appear normal.

Liver is mildly enlarged (cranio-caudal dimension approximately 16 cms). Margins are


regular. Parenchyma shows normal and uniform attenuation values. There is no
intrahepatic biliary dilatation.

Gall bladder is partially distended. Walls are regular & smooth. No definite intra mural or
intra luminal abnormality is seen. CBD is not dilated.

Pancreas normal in size. Margins are regular. Parenchyma shows normal and uniform
density. Pancreatic duct is not dilated. No focal area of altered density or calcification is
seen. Peripancreatic fat planes are preserved.
Spleen is normal in size. Margins are regular with uniform parenchymal density.

Kidneys both kidneys are normal in position and size. Margins are regular. Parenchymal
thicknesses are adequate with normal nephrographic density. No evidence of
backpressure changes are seen in the pelvicalyceal system. Both the ureters are seen in
their entire extent displaying normal course and calibre.

Calcifications seen involving walls of the aorta. No lymphadenopathy.

Urinary bladder is distended. Walls are mildly thickened. Perivesical fat planes clear.

Prostate is enlarged (~ 32 gms), showing normal density and outline. No evidence of focal
abnormality or sizable mass lesion noted.

No free fluid is seen in the peritoneal cavity.

IMPRESSION : Follow up case of right lung mass –


• CT scan revealed heterogeneously enhancing soft tissue density lesion with irregular
margins involving right hila encasing, mildly narrowing right upper lobe, right lower
lobe bronchus, right pulmonary artery with extension in lung parenchyma and
enlarged mediastinal lymphnodes with minimal right pleural fluid. Compared with
previous CT scan dated 01-11-2021, showed decrease in size of the lesion and
lymphnodes.

• Mild hepatomegaly.

• Enlarged prostate (~ 32 gms) with mildly thickened urinary bladder walls.


Note: Gall bladder calculi are not visualised on CT scan in 90% cases, correlation with ultrasonography is
advisable.

(Please correlate clinically & with other investigations.)
