HISTORY : Patient has complaint of right flank pain since the past 15 days. Right renal mass in addition to a right
renal calculus was seen on non-contrast CT OSH.
Primary physician has requested CT abdomen with and without contrast (renal mass protocol) for
characterization of this mass.
REPORT : CT abdomen with and without intravenous contrast.
Findings:
The right renal lower pole exophytic heterogeneously enhancing mass measures 6.2 x 6.3 x 7.3 cm. It
is demonstrating internal areas of necrosis and extension of disease into the perinephric fat and
neovascularity. There is no venous invasion. The internal density of renal mass is 25, 88 and 106 HU
on the unenhanced, corticomedullary and nephrographic phases respectively. These findings are
suggestive of clear cell renal cell cancer.
The adrenals and left kidney are within normal limits. The spleen is not enlarged. No focal hepatic
lesion or biliary ductal dilation is noted. The gallbladder is contracted. The portal vein is patent.
Subcentimeter retroperitoneal lymph nodes are seen. The visualised bowel is not dilated. The visualised
appendix is normal. Dependent atelectasis in the lungs is noted. The bone windows are unremarkable.
CONCLUSION : T2 N0 right renal cell cancer (likely clear cell variant).
